Project Name and Location: Groundwater Monitoring and Reporting In accordance with the original Professional Services Agreement dated 7 October 2003 and Change Orders thereto, the Agreement changes as detailed below are hereby authorized.
This change order has been prepared and authorizes groundwater reporting services in three-year increments.
Accordingly, beginning with the fourth quarter/annual of 2021 report, this addendum will expire with the completion of the fourth quarter/annual report for 2024 groundwater monitoring report and by February 26, 2025.
Task 1: Continued Groundwater Monitoring and Reporting for the WWTF Note that this change order task only extends the contract time for preparation of your Regional Board required quarterly and annual groundwater monitoring reports for the City’s WWTF. We propose to prepare your quarterly reports for a fee of $2,150 (1st, 2nd, and 3rd quarters) and the annual report for a fee of $5,000 (yearly total of not to exceed $11,450 per year for a total through the fourth quarter 2024 report of $39,350). See breakdown of fees below.

Effect on Schedule: None
Payments shall be made in accordance with the original agreement terms. All other items and conditions of the original Agreement shall remain in full force and effect.
