PROJECT: Stockton Avenue Rehabilitation Project SHEET 1 OF 1 SHEETS TO: D.A. Wood Construction, Inc.
You are hereby directed to make the herein described changes from the plans and specifications or do the following described work not included in the plans and specifications on this contract.
NOTE: This change order is not effective until approved by the CITY ENGINEER.
Description of work to be done, estimate of quantities, and prices to be paid. Segregate between additional work at contract price, agreed price and force account.
Unless otherwise stated, rates for rental equipment cover only such time as equipment is actually used and no allowance will be made for idle time.
Change requested by the Engineer.
Description Cost
Addition of the following items:
1. Cost of regrading, repaving, and providing drainage to 733 and 737 South Stockton Avenue $67,083.64
2. Cost of Concrete Price Escalations $24,561.00
Total: $91,644.64
All work shall be in accordance with City Standards.
Estimated Cost: $91,644.64 Increase
By reason of this order the time of completion will be adjusted as follows: (0) working days for the work associated with Change Order 11, in addition to (15) working days for Change Order 1 work, (49) working days for Change Order 2 work, (17) working days for Change Order 3 work, (5) working days for Change Order 4 work, (6) working days for Change Order 5 work, (0) working days for Change Order 6 work, (5) working days for Change Order 7 work, (0) working days for Change Order 8 work, (0) working days for Change Order 9 work, (0) working days for Change Order 10, and (0) working days for Change Order 11. Total number of additional working days added is 97.
