PROJECT: Stockton Avenue Rehabilitation Project SHEET 1 OF 1 SHEETS TO: D.A. Wood Construction, Inc.
You are hereby directed to make the herein described changes from the plans and specifications or do the following described work not included in the plans and specifications on this contract.
NOTE: This change order is not effective until approved by the CITY ENGINEER Description of work to be done, estimate of quantities, and prices to be paid. Segregate between additional work at contract price, agreed price and force acoount Unless otherwise stated, rates for rental equipment cover only such time as equipment is actually used and no allowance will be made for idle time.
Change requested by the Engineer.
Description Cost
Addition of the following items:
1. Balancing Line Items and cost of railing and paving fixes -$38,845.81
2. Cost of repairs during work (base rock, water line hazard, regrading, raising iron) $40,273.18 Total: $1,427.37
All work shall be in accordance with City Standards.
Estimated Cost: $1,427.37 Increase
By reason of this order the time of completion will be adjusted as follows: (0) working days for the work associated with Change Order 13, in addition to (15) working days for Change Order 1 work, (49) working days for Change Order 2 work, (17) working days for Change Order 3 work, (5) working days for Change Order 4 work, (6) working days for Change Order 5 work, (0) working days for Change Order 6 work, (5) working days for Change Order 7 work, (0) vorking days for Change Order 8 work, (0) working days for Change Order 9 work, (0) working days for Change Order 10 work, (0) working days for Change Order 11 work and (0) working days for Change Order 12 work. (Total number ol additional working days added is 97.)
