Project Name and Location: Groundwater Monitoring and Reporting In accordance with the original Professional Services Agreement dated 7 October 2003 and Change Orders thereto, the Agreement changes as detailed below are hereby authorized.
This change order has been prepared and authorizes groundwater reporting services in three-year increments.
Accordingly, beginning with the first quarter 2025 report, this addendum will expire with the completion of the fourth quarter/annual report for 2027 groundwater monitoring report and by February 29, 2028.
Task 1: Continued Groundwater Monitoring and Reporting for the WWTF Note that this change order task only extends the contract time for preparation of your Regional Board required quarterly and annual groundwater monitoring reports for the City’s WWTF. We propose to prepare your quarterly reports for a fee of $2,500 (1st, 2nd, and 3rd quarters) and the annual report for a fee of $5,500 (yearly total of not to exceed $13,000 per year for a total through the fourth quarter 2027 report of $39,000).
TOTAL CHANGE ORDER AUTHORIZATION: $39,000
Effect on Schedule: None
Payments shall be made in accordance with the original agreement terms. All other items and conditions of the original Agreement shall remain in full force and effect.
