You are receiving this letter and the accompanying document because you are designated as a remainder beneficiary to the Paula M. Carroll Trust dated March 14, 2000, as subsequently amended, including the Sixth Amendment to the Paula M. Carroll Trust executed January 26, 2023. The Trust is to be terminated, and all assets distributed pursuant to the terms of said Trust, as amended. The distributions involved are detailed within the enclosed five-page document entitled "THE PAULA M. CARROLL TRUST — DISTRIBUTION AGREEMENT" (the Agreement).
Please review the Agreement and, if acceptable, sign and date the Agreement on Page 5 under the appropriate signature block that corresponds with your beneficial interest. Thereafter, the signed form may be mailed or personally delivered to the following address:
