We hereby subm
it specifications and estimates for:
(1) Existing on-site Equipment provided by Mutual Propane – (2) 1,999 Gallon Horizontal ASME tanks. (2) Existing Foundations in place
(3) Service Co ntract will be $1 per year. Mutual will maintain the tank in proper working condition. Mutual will invoice customer $1.00 for the tank, fill hose and necessary fittings and thereafter own the same.
(4) Customer will b e filling:
Forklifts Containers with POL / OPD fitting Vehicles with ACME 1-3/4 inch adapter Other (Specify) (5)= Propane price will be $01.712 per gallon, plus applicable sales taxes. (6) Compliance fee: Our vehicles and drivers meet all state and federal laws regulating our industry. $7.25 per delivery. (7) Existing Crash Protection is in place.

(9) Customer is responsible for the code required fire extinguisher installed at propane tank location, or one mounted on each forklift, if applicable. (10) Additional comments:
All material is guaranteed to be as specified. All work to be completed in a workmanlike manner according to standard practices. Any alteration or deviation from above specifications, including under ground obstacles, involving extra costs will be executed only upon written orders, and will become an extra charge over and above the estimate. All agreements contingent upon strikes, accidents or delays beyond our control. Owner to carry fire, tornado and other necessary insurance. Our workers are fully covered by Workers’ Compensation Insurance.

Acceptance of Proposal – The above specifications and conditions are satisfactory and are hereby accepted. This agreement shall continue in force and be binding upon the parties for a period of 1
year(s) from acceptance date, and
thereafter from year-to-year unless cancelled by either party. However, propane prices may be adjusted upward or downwa rd periodically depending on cost of product. After period end, any cancellation shall be by sixty-day written notice sent to the main business address of the party to be notified. You are authorized to do the work specified.
