Alyssa is out of the office today, so I am responding on her behalf. If you will be coming into City Hall to file an appeal, please note that the Planning counter (the One Stop Shop) closes at 4pm, so you will need to arrive at City Hall before such time, to process the appeal.
There is no longer a referral form that needs to be completed. If a Councilmember wishes to refer the case to a public hearing before Council, they can email their request (by 5pm today) to my attention, or anyone else that is copied on this email.

There may be a request from a Councilmember coming in tomorrow for a referral to the Council of the DRC approval of this case – if not, an appeal will be filed in the afternoon of Monday, Oct. 3. Is it correct that the Planning office will be open for that until 5:00 pm?
Copied below is some correspondence with then Councilmember Gardner about a referral to the Council of a DRC approval of a warehouse project in 2018. He refers to executing a form and sending it to the case planner, Candice Assadzadeh in that case.
Please email me a copy of any such blank form for a referral, and please let me know, in case the Councilmember wishes to request the deferral and submit the form, to whom at the City should the form be submitted – you in the present case? – and which City officials should be copied on it.

Thank you for your email. This email is to confirm that I received your referral form for Planning Cases P17-0506, P17-0507, P17-0748 and P17-0749. On referral, the project will be scheduled for the Council’s discussion calendar, with an anticipated hearing date of Tuesday, June 19th.
Please note that should an appeal be received by staff, the project will be scheduled for a public hearing before Planning Commission, as the Zoning Code identifies Planning Commission as the appeal authority for Design Review cases.
