Description/Analysis
Issue Detail:
Emergency Medical Services (EMS) systems are regulated and governed by the California Health and Safety Code, Division 2.5, Section 1797-1799.207, Title 22 of the California Code of Regulations, the California Emergency Medical Services Authority (CEMSA), and the Sacramento County EMS Agency (SCEMSA). To meet the regulations and requirements set forth by these governing bodies to deliver Advanced Life Support (ALS) and Basic Life Support (BLS) Emergency medical services in the City of Sacramento and our contracted communities, the Sacramento Fire Department (SFD) is required to maintain the services of a licensed physician, often referred to as a “Medical Director” (MD).
In previous years, the SFD acquired those services through a contract with the Sacramento Regional Fire/EMS Communications Center (SRFECC). As of June 30, 2022, the SRFECC decided not to renew those contracted services with each participating agency (SFD, Sacramento Metro Fire District, Consumnes Fire Department and Folsom Fire Department). In light of SRFECC’s decision,

each fire agency had to contract services with their own MD independently.
The SFD executed a two (2) year contract with Permanente Medical Group, Inc. (PMG) from July 1, 2022, to June 30, 2024. Council approval is required to execute a one-year extension to the existing contract with PMG for Medical Director Services through June 30, 2025 to remain in compliance with the regulations set by the governing authorities.
Policy Considerations: The initial contract was executed under Sacramento City Code section
3.56.080 - Authority of City Manager. Subject to the availability of funds and the procedures set forth in Chapter 3.56, the City Manager is authorized to bind the city, by written contract or purchase order, involving an expenditure under $250,000 in any one transaction, without first advertising or obtaining specific authorization by the City Council, for the purchase of supplies or nonprofessional services for the city.
This council report requests an increased not-to-exceed amount requiring Council approval per the Sacramento City Code section 3.56.090, all contracts for supplies or nonprofessional services involving an expenditure of $250,000 or more shall be awarded by the City Council. Such contracts shall be formal written agreements executed by the City Manager on behalf of the city. The signature by the City Manager constitutes his or her certification that there remain unexpended and unapplied balances of the appropriations or funds applicable to the contract sufficient to pay the estimated expense of executing the contract.
Economic Impacts: None.
Environmental Considerations: Ongoing administrative maintenance activities, such as purchases of supplies, equipment or materials which are not made for purposes of a public construction project, do not constitute a “project” and are exempt from the California Environmental Quality Act (CEQA) CEQA Guidelines, Section 15061(b) (3), 15378(b) (2).
Sustainability: Not Applicable.
Commission/Committee Action: Not Applicable.
Rationale for Recommendation: Staff is recommending the approval of a one-year contract extension between the City of Sacramento Fire Department and Permanente Medical Group, Inc. (PMG) to staff the Medical Director position from July 1, 2024, through June 30, 2025. The City of Sacramento must meet regulations and requirements of having a Medical Director as described in the above mentioned issue. When Medical Director services were provided by the SRFECC, the provider was PMG. The licensed physicians provided by PMG have a diverse and extensive background in pre-hospital emergency medical services (EMS) systems. In addition, they are familiar with the regulations required by the governing authorities to remain in compliance to provide EMS in Sacramento County.
File ID: 2024-01264 6/25/2024 Consent Item 22.
Financial Considerations: Sufficient funding is available in the Fire Department’s Fiscal Year (FY) 2024/25 operating budget to pay for services through June 30, 2025.
Local Business Enterprise (LBE): Not Applicable.
