Proposal for: Limited Hazardous Materials Survey
Burned Down Vacant Woolworth Building
Corner of 4th Street & E. Street.

San Bernardino, California 92408

Dear Mr. Alkhateeb:

Envirocare Consulting, Inc (ECI) is pleased to provide The City of San Bernardino with our proposal to perform a Limited Hazardous Material Survey for the above -referenced property. Our proposal contains: a summary of relevant information as we understand it, a project schedule, and the estimated costs for completion of the proposed work.

PROJECT OVERVIEW
ECI understands the existing burned-down structures (Woolworth building) located at the corner of 4th Street & E. Street in the city of San Bernardino, California is planned for renovation and/or demolition.

ECI proposes to perform a Limited Hazardous Material Survey including an asbestos-containing material (ACM) survey.

SCOPE OF SERVICES
Envirocare Consulting, Inc. will provide a State of California Division of Occupational Safety and Health Certified Asbestos Consultant to perform Asbestos survey according to (Cal/OSHA, Cal/EPA and SCAQMD protocol rules 1403).

Envirocare Consulting, Inc. will conduct an assessment of the site and materials while exclusively collecting accessible samples (meaning the sampled materials are able to be reached or gathered safely despite the fire - damaged condition of the Woolworth building) according to the scope of services and requirements, all asbestos samples will be submitted to and analyzed by a laboratory which is accredited by NVLAP and Cal/EPA’s program. All samples will be analyzed by PLM.

Envirocare Consulting, Inc. will take digital photographs of suspected sampled materials.

Envirocare Consulting, Inc. will summit a final report including sample locations, sample results, quantity estimation and photographs.

3 The asbestos -containing material (ACM) Assessment & Survey will be conducted by inspectors with EPA accredited asbestos training to identify asbestos -containing materials in general accordance with EPA Standard 40 CFR 763, Sub Part E, Asbestos Hazard Emergency Response Act (AHERA) and OSHA Standard 29 CFR 1926.1101 Inspection Protocol. Th is survey and assessment include the collection of bulk samples of friable (easily crumbled under hand pressure) and non -friable materials suspected as containing asbestos for laboratory testing as well as an initial walkthrough and visual analysis of the site and materials.

Due to the nature of an asbestos assessment and the inability and impracticality of accessing all hidden locations: some areas/materials may still be deemed inaccessible and/or not inspected.

Representative bulk samples will be collected from each suspect material within the inspected areas in general accordance with EPA recommended asbestos survey techniques. In instances where it is apparent that the materials are not of homogenous appearance and texture, individual samples will be collected.

All of the collected samples will be submitted to a laboratory for analysis by polarized light microscopy (PLM) in accordance with EPA testing methods. Samples will be analyzed by EPA method (EPA 600/4 -93/116) Determination of Asbestos in Bulk Building Mat erials or EPA Method (EPA 600M4 -82-020) for the Determination of Asbestos in Bulk Insulation Samples. The laboratory is accredited by the U.S. Department of Commerce, National Institute of Standards and Technologies, National Voluntary Laboratory Accredita tion Program (NVLAP) for bulk asbestos identification by PLM.

The results of the asbestos bulk sampling will be included in a written survey report detailing pertinent observations made while on-site and estimated quantities of ACMs and recommendations for removal. EC I will rely on your office to facilitate the necessary site access arrangements for the completion of the proposed scope of services.

Schedule: Our proposed scope of work can be initiated after we receive formal authorization to proceed as well as a signed purchase order . We anticipate that the proposed investigation for the subject site can be completed within Five-(5) working days of authorization to proceed. This letter is the agreement for our services. Your acceptance of this proposal may be indicated by signing and returning the enclosed copy to us. If you have any questions or comments, please feel free to contact us.
