Overview
This Change Order #5 is a change to the Agreement between City of San Bernardino CA (“Client” and TruePoint Solutions, dba: GovPath (“Provider”) signed October 28th, 2024.
Description of Additional Services
Additional services are required to complete solution-specific discovery, design validation, system configuration, testing, and implementation activities. Due to changes in project direction and timing, the originally planned schedule windows for these activities have already passed, requiring the project team to revisit and re-execute portions of the analysis, design, and implementation effort to successfully deliver the solution. Services will include:
• GWiz requirements analysis and discovery
• Business process and workflow review
• Solution design validation
• GWiz configuration and implementation
• Stakeholder review and feedback sessions
• System testing and validation
• Configuration refinement and issue resolution
Client Responsibilities
• Designate project stakeholders and subject matter experts (SMEs) to participate in GWiz discovery, requirements gathering, design review, and testing activities. • Provide timely access to required business process documentation, forms, workflows, data elements, and other information necessary to support solution design and configuration. • Participate in requirements validation and configuration review sessions and provide feedback within agreed-upon timeframes.
• Review and approve documented business requirements, configuration decisions, and design recommendations.

• Identify and communicate business rules, operational procedures, and reporting requirements necessary for GWiz implementation.
• Support testing and validation activities, including execution of user acceptance testing and verification of configured functionality.
• Provide consolidated feedback, issue prioritization, and approval of defect resolutions during validation activities.
• Complete all requested reviews, decisions, and approvals in a timely manner to avoid impacts to project schedule and delivery timelines.
• Ensure appropriate staff availability and participation throughout implementation and validation activities.
• Provide final acceptance of the configured GWiz solution upon successful completion of testing and validation.
• The Customer is responsible for providing resources and participation necessary to complete User Acceptance Testing (UAT), deployment readiness reviews, and production go-live preparation activities as scheduled in the current project plan. Additional services may be required if these activities occur outside of the approved project timeline.
Cost Changes
This Change Order adds 300 hours of professional services at a rate of $195 per hour to support GWiz discovery, configuration, testing, and implementation activities, for a total cost increase of $58,500 billed time and materials.
