The City of San Leandro, incorporated in 1872, has a rich history rooted in early Native American habitation by the Ohlone people. During the Spanish and Mexican periods, the area was part of Rancho San Antonio, granted to Don Luis Peralta in 1820. Following California’s statehood, San Leandro developed as an agricultural hub, known for cherries, grains, and dairy production. Its proximity to the San Francisco Bay and growing rail connections supported industrial growth in the early 20th century. Over time, San Leandro evolved into a diverse suburban community with a strong mix of residential neighborhoods, light industry, and commercial development, while maintaining its historic downtown and cultural heritage.

Project Information
Project Title San Leandro Senior Grocery Delivery Program Please provide a 1-2 sentence snapshot of your
project, following the format exactly in the
provided directions.

The City of San Leandro is seeking up to $25,000 to support a senior grocery delivery program for homebound older adults by providing twice monthly delivery of healthy food and basic necessities and connecting participants to additional health and social services.

In a paragraph or less, please describe your
organization’s strengths to successfully achieve
the work described in the proposal.

The City of San Leandro is well positioned to successfully implement the proposed program through its strong organizational capacity, established partnerships, and commitment to supporting older adults. The City recently adopted its Age Friendly Action Plan, which provides a clear framework for advancing health and wellness initiatives for seniors and reflects extensive community engagement. Through its Senior Services Division and collaboration with local nonprofit partners, the City has experience delivering programs that address food access, social isolation, and basic needs for vulnerable populations. The proposed program builds directly on these efforts and aligns with the City’s broader goal of helping residents age in place with dignity. San Leandro’s ability to coordinate services, leverage partnerships, and respond to community identified needs ensures effective implementation and meaningful impact.

Outcomes & Metrics
How many outcomes would you like to add for
this grant?
1 Outcome 1 This grant will advance the outcome of increasing the number of bags of groceries delivered to homebound older adults in San Leandro. Through twice monthly deliveries, the program will distribute approximately 40 grocery bags per month, reaching up to 80 unduplicated households annually. This outcome reflects a measurable increase in access to healthy food for seniors who are unable to visit traditional food distribution sites, directly supporting improved nutrition, health, and the ability to safely age in place.

Please indicate the number of metrics you will
use to track and measure progress toward

Reach & Project Plan
Families: How many unique families will be
directly served by this funding?
80 Would you like to report an additional reach
unit?
No
Please select the Community Health Need
which your project will address.

Income & Employment
Please provide an overview of the selected
community health need.

Access to healthy food remains a critical community health need for low income and homebound older adults in San Leandro and the broader Alameda County region. Many seniors live on fixed incomes and face barriers such as limited mobility, chronic health conditions, and lack of transportation, making it difficult to access grocery stores or food distribution sites. Through the City’s Age Friendly Action Plan outreach process, residents identified food access as a key concern, particularly for those who are homebound or living with disabilities. Alameda County is home to a growing senior population, and a significant portion experience food insecurity or are at risk due to rising costs of living and limited access to affordable, nutritious food. Without targeted interventions, these barriers can lead to poor nutrition, worsening health outcomes, and increased social isolation among vulnerable older adults.

How many objectives are included in your
project plan?
1 Objective 1 By June 2027, the City of San Leandro will deliver at least 480 bags of groceries to homebound older adults through twice monthly distributions, increasing access to healthy food for a minimum of 80 unduplicated households.

Please provide an overview of the proposed
project activities for each project objective.

To achieve the objective of delivering groceries to homebound older adults, the City of San Leandro will implement a coordinated set of activities in partnership with a local nonprofit provider. The program will begin with participant identification and enrollment, prioritizing low income seniors aged 50 and older who are homebound due to mobility limitations or medical conditions. Program staff will conduct initial assessments to determine eligibility and identify additional service needs.

Once enrolled, participants will receive twice monthly deliveries of grocery bags containing fresh produce, shelf stable food, and essential items. Staff will procure and package groceries, coordinate delivery routes, and ensure deliveries are completed within a two hour window to maintain food quality. During deliveries, staff will also conduct wellness check ins and provide referrals to health and social services as needed.

The City will track program outputs, including the number of grocery bags delivered and households served, and will collect participant feedback to assess satisfaction and inform program improvements.

Please select the city or cities within the Kaiser
Permanente service area where your project
activities will take place.

If your organization had a deficit, please explain
why
The City’s General Fund deficit is driven by rising expenditures associated with employee salaries and benefits (e.g., pension and Other Post Employment Benefits), cost increases in goods and services to deliver essential municipal services, and the increased costs associated with the deferred maintenance of city assets. These expenditure requirements continue to outpace the growth of revenues.
