You are hereby directed to make the herein described changes from the plans and specifications or do the following described work not included in the plans and specifications on your contract.
DESCRIPTION OF WORK
This change order will extend Enterprise Automation’s (EA) contract to maintain and improve the integrated Supervisory Control and Data Acquisition System (SCADA). The original contract and scope only covered sewer and stormwater collection systems scope of work. Change Order No. 1 expanded the scope to include an evaluation and implementation of additional functionality improvements to the SCADA system at wastewater treatment plant (WWTP). Change Order No. 2 was a no-cost time extension. Change Order No. 3 extended EA’s scope to continue providing WWTP SCADA system maintenance support for the fiscal year 2024-25.
Change Order No 4 will extend EA’s scope to cover Fiscal Year 2025-26, as follows:
Revise “Section 5 - TERM” of the Agreement as follows: “The services to be performed under this agreement shall commence upon execution of this agreement and be completed on July 31, 2026.” Revise the “Exhibit A Scope of Work” of the Agreement to include “SCADA Support Contract FY 2025/2026”.
Revise the second sentence of “Section 6 - Payment', 6.1a” of the Agreement as follows: "In consideration for the full performance of the services set forth in Exhibit A , customer agrees to pay vendor in an amount up to $475,679 pursuant to fee rates stated in Exhibit A.”
Total cost of change not to exceed: $125,000
We, the undersigned contractor, have given careful consideration to the change proposed and hereby agree, if this proposal is approved, that we will provide all equipment, furnish all materials, except as may otherwise be noted above, and perform all services necessary for the work above specified, and will accept as full payment therefore the prices shown above.
