Scope and Cost Amendment for the 19th Avenue Channel Project 3 Onterris is pleased to provide this scope amendment to support environmental compliance and permitting for maintenance work in the 19th Avenue Channel in the City of San Mateo. Onterris, formerly Montrose, was retained in October 2025 to support environmental compliance and permitting for the 16th Avenue Channel site. Following a large rain event during the late 2025 and early 2026 storm season, the City requested that Onterris expand its support to include the 19th Avenue Channel site. To address priority maintenance needs at both locations, the 16th Avenue Channel and 19th Avenue Channel sites were combined into the City of San Mateo’s 2026 High Priority Flood Risk Reduction Projects. The combined project is intended to remove vegetation overgrowth and restore stormwater conveyance capacity within both channels.

To support implementation during the 2026 work period, Onterris has completed CEQA review and prepared permit applications for both sites under the 2026 High Priority Flood Risk Reduction Projects. This amendment provides additional scope of work elements and budget to cover the work requested by the City to incorporate the 19th Avenue Channel site into the environmental review and permitting effort, including the environmental documentation, permitting support, and agency coordination needed to include the site in the 2026 implementation package. The detailed scope of work below presents the original 16th Avenue Channel scope, with added language shown in blue to identify the completed tasks and ongoing support associated with incorporating the 19th Avenue Channel site into the 2026 High Priority Flood Risk Reduction Projects.

Scope of Work
Task 6. Project-Specific Permit Application: 16th Avenue and 19th Avenue Channels The 16th Avenue Channel Project involves cutting instream vegetation at the root-line or waterline within a concrete channel and directly placing the vegetation in a dump truck along the top of bank; no ground disturbance would be required. Thus, this task assumes that the project would be exempt from a CWA Section 404 permit from USACE under the USACE’s 2008 Final Ruling. If a CWA Section 404 permit is required by the USACE, this scope and cost would be revised accordingly.

Following the City’s request to expand the environmental compliance and permitting effort, this task has been updated to include the 19th Avenue Channel site as part of the City of San Mateo’s 2026 High Priority Flood Risk Reduction Projects. The 19th Avenue Channel maintenance work includes removal of herbaceous vegetation within a concrete-lined flood control channel to restore conveyance capacity and reduce localized flood risk. Because the 19th Avenue Channel work includes equipment access within the channel and may require temporary dewatering if water is present during construction, additional environmental review, permit application materials, impact calculations, best management practice descriptions, and regulatory agency coordination were required to incorporate the site into the 2026 implementation package.

Task 6.1. Categorical Exemption
Task 6.1 assumes that the 16th Avenue Channel Project qualifies for a Categorical Exemption under CEQA Guidelines Section 15301 (Existing Facilities) as the project involves routine maintenance of an existing facility and no significant environmental impacts are anticipated. Montrose will prepare a memorandum that will identify the exemption, explain why the project qualifies for the exemption, and confirm that no exceptions under CEQA Guidelines Section 15300.2 apply. Montrose will submit the draft memorandum to the City for one round of review and comment and will revise it Exhibit A-1
City of San Mateo
Scope and Cost Amendment for the 19th Avenue Channel Project 4 based on the City’s feedback.

Montrose will also prepare a draft Notice of Exemption (NOE) and submit it to the City for one round of review and comment. This task assumes that the City will be responsible for filing the NOE with the State Clearinghouse and San Mateo County Clerk and paying associated fees.

If a Categorical Exemption is determined not to be the appropriate CEQA document for this project, the scope and cost will be revised accordingly.

As part of the expanded 2026 High Priority Flood Risk Reduction Projects effort, Onterris evaluated the 19th Avenue Channel site for inclusion under the same CEQA compliance approach as the 16th Avenue Channel site. This work included updating the project description to incorporate the 19th Avenue Channel site, reviewing proposed work methods and site conditions, evaluating potential environmental constraints, and addressing both sites in the Categorical Exemption memorandum and supporting CEQA documentation. Onterris also prepared an NOE for the 2026 High Priority Flood Risk Reduction Projects, which included work at both the 16th Avenue and 19th Avenue Channel sites.

Task 6.1 Deliverables
Draft and final Categorical Exemption memorandum (electronic) Draft and final Notice of Exemption (electronic)
Task 6.2 San Francisco Bay RWQCB Waste Discharge Requirements Permit Montrose will prepare a permit application package for an authorization of Waste Discharge Requirements (WDRs) from the San Francisco Bay RWQCB for the 16th Avenue Channel Project.

Because we are assuming that the project will be exempt from Section 404 jurisdiction of the USACE, we also assume that the project will not require a Section 401 Water Quality Certification but will require the WDR authorization under State requirements. The application package will include a cover letter, Form 200 Report of Waste Discharge, project description, impacts to water bodies, BMPs, and CEQA documentation. A draft application package will be submitted to the City for one round of review and revision. Prior to submitting the application package to the RWQCB, Montrose will request a pre-filing meeting with the RWQCB; the RWQCB will determine whether a meeting is necessary. Montrose will also support responses to RWQCB requests for supplemental information during the review process, as budget allows. This scope assumes that the City will be responsible for paying RWQCB application fees.

Under this scope amendment, Onterris prepared additional RWQCB application materials under this task for the 19th Avenue Channel site, addressing work within the concrete-lined channel, potential temporary dewatering, equipment access within the channel, staging along Pacific Boulevard, temporary and permanent impact areas, and applicable BMPs to avoid and minimize water quality impacts.

A draft application package was submitted to the City for one round of review and revision. Prior to submitting the application package to the RWQCB, Onterris requested and participated in a pre- filing meeting with the RWQCB. Following the meeting, Onterris supported the City in responding to questions from Jackson regarding the 401 Water Quality Certification application. Onterris will continue to support responses to RWQCB requests for supplemental information, as needed, to obtain the 401 Water Quality Certification.

One (1) pre-filing meeting with the City and RWQCB
Task 6.2 Deliverables
Draft and revised RWQCB WDR permit application (electronic) Pre-filing meeting materials (electronic)
Task 6.3 CDFW Section 1602 Lake and Streambed Alteration Agreement Montrose will develop a standard application for a Lake and Streambed Alteration Agreement (LSAA) under Fish and Game Code Section 1602 for the 16th Avenue Channel Project. The application package will follow the CDFW Environmental Permit Information Management System (EPIMS) External Permitting Portal application format. Montrose will also prepare supplemental information, including a project description, construction BMPs, and other maps and figures as needed to support the application. A draft application package will be submitted to the City for one round of review and revision. This scope of work assumes that the City will open an application in the EPIMS system to initiate the application process and add Montrose’s project manager, allowing Montrose to upload the application materials on the City’s behalf. The City will be responsible for application fees and final submittal through EPIMS.

Under this scope amendment, this task was expanded to include the 19th Avenue Channel site in the CDFW notification package. Additional work included incorporating the 19th Avenue Channel project description, work limits, construction methods, access and staging information, potential temporary dewatering, impact calculations, and applicable avoidance and minimization measures into the CDFW submittal. Onterris coordinated with CDFW regarding the combined 16th Avenue and 19th Avenue Channel work under the 2026 High Priority Flood Risk Reduction Projects and supported responses to CDFW questions regarding the 19th Avenue Channel site, as needed. The CDFW 1602 permit has been acquired for the combined project.

Task 6.3 Deliverables
Draft and revised CDFW permit application package (electronic) Task 6.4 USACE Section 404 Clean Water Act
The original scope assumed that the 16th Avenue Channel maintenance work would not require authorization from USACE under Clean Water Act Section 404 because the work was limited to cutting instream vegetation at the root-line or waterline within a concrete channel and did not involve ground disturbance. However, following the City’s request to add the 19th Avenue Channel site to the 2026 High Priority Flood Risk Reduction Projects, USACE Section 404 authorization was required for the 19th Avenue Channel work.

Under this scope amendment, Onterris prepared a USACE Section 404 permit application package for the 19th Avenue Channel site. The application package included the project description, work limits, construction methods, access and staging information, potential temporary dewatering City of San Mateo
Scope and Cost Amendment for the 19th Avenue Channel Project 6 approach, impact calculations, BMPs, and supporting environmental documentation. Onterris submitted the application package to USACE and coordinated with USACE during the review process. USACE has since issued a provisional Section 404 permit for the 19th Avenue Channel work, pending receipt of the RWQCB Section 401 Water Quality Certification.

Task 6.4 Deliverables
Draft and revised USACE permit application package (electronic) Task 6.5 Regulatory Agency Coordination
Montrose will support the City with additional agency coordination for the 16th Avenue Channel Project. This includes tracking permit review and approval timelines; responding to agency questions regarding application materials; providing project updates; preparing written responses or memoranda to address regulatory requests; and participating in calls, meetings, or site visits with agency staff as needed. The budget estimate for this task is based on Montrose’s experience with similar projects. Montrose will provide regulatory coordination services to the level of effort outlined in the cost estimate for this task.

This task has been expanded to include regulatory agency coordination for the 19th Avenue Channel site under the 2026 High Priority Flood Risk Reduction Projects. Additional coordination has included communications with RWQCB, CDFW, USACE, and City staff regarding the 19th Avenue Channel site; tracking permit review timelines; preparing responses to agency questions; updating permit materials to reflect agency feedback; and participating in calls or meetings necessary to support timely authorization of the 2026 implementation package.

As of July 8, 2026, CDFW has issued the Section 1602 permit, and USACE has issued a provisional Section 404 notification pending receipt of the RWQCB Section 401 Water Quality Certification.

Under this scope amendment, Onterris will continue to coordinate with the RWQCB, as needed, to support issuance of the Section 401 Water Quality Certification.

Budget
Onterris shall complete the additional 19th Avenue Channel scope of work on a time-and- materials basis for a not-to-exceed amount of $40,956. The updated level of effort associated with each task is provided in Exhibit A. The total change order amount for the additional 19th Avenue Channel work will not exceed $40,956 without prior written authorization from the City.
