Service Agreement Number 100859 is hereby amended as follows:
Increase the estimated cost to SFPUC by $16,800 for labor and/or materials supplied by the JPB, its contractors and consultants in connection with the fully executed Service Agreement pertaining to City of San Mateo Villa Terrace and E Bellevue Ave Grade Closure.
Amendment 1 will include review of 100 percent drawings for IFB.
Original Estimate $59,440
Amendment # 1 $16,800
Total Revised Estimate $76,240
Please submit a check in the amount of $16,800 for the additional budget. Please mail check payment to:
