AMENDMENTTO AGREEMENTBETWEEN CITY OF SANPABLOAND CONTRACOSTA HEALTHSERVICES (
RE HEALTH, HOUSING AND HOMELESSSERVICES) V
1 THIS AGREEMENT ("Agreement") is madeand enteredinto as of December1, 2020, by andbetween the Cityof SanPablo (hereinafterreferred to as the "City"), and the Countyof ContraCosta U. ( acting throughits HealthServicesDepartment, Health, Housingand HomelessServices Division, 0 hereinafter
referred to as "Contractor"). RECITALS
WHEREAS,
the partiesenteredinto an agreement ("Original Agreement") dated December1, 2018, under which the Contractoragreed to providespecifiedservices for homelesspersons in theU City
in exchangefor whichthe Cityagreedto payContractorcertain sums of money; and WHEREAS, the partieshave determinedthat the servicesprovided by Contractorunder the Original Agreementwere efficaciousand beneficialto theCity's homelesspopulation, and, thus, the parties desire to extendthe OriginalAgreement's termto andincludingNovember30, 2021: NOW, THEREFORE, IN CONSIDERATIONof themutualcovenantsand promisesof thepartiesherein contained, the partieshereto agree as follows: AGREEMENT
1. Pursuantto section1 oftheFirst Amendmentto theOriginalAgreement, the termof theOriginal Agreementshall be extendedtwelve months to andincludingNovember 30, 2021. 2.
Section3( A) of theOriginalAgreementis amendedto readas follows: A.
CommencingDecember 1, 2020, Contractorshall be paidon an annualbasis in arrearsafter serviceshave been renderedand documentedin an invoicenot to exceed $ 25, 000 over the termof thisAgreement. The paymentwill includethe HousingAssistanceFee of $ 1, 250. 3.
In allotherrespectsnot amendedby thisAgreement, the OriginalAgreementshall remainin full forceand effect. IN
