NOTICE TO APPLICANTS, BIDDERS, PROPOSERS
AND OTHERS SEEKING DISCRETIONARY PERMITS, CONTRACTS,
OR OTHER BENEFITS FROM THE CITY OF SANTA MONICA
Sonto Monico's voters odopted o City Chorter omendment commonly known os the Ooks Initiotive. The Ooks Initiotive requires the City to provide this notice ond informotion obout the Initiotive's requirements. You moy obtoin o full copy of the Initiotive's text f rom the Citv Clerk.
This informotion is required by City Chorter Article XXll-Toxpoyer Protection.
It prohibits o public officiol from receiving, ond o person or entity from conferring, specified personol benefits or compoign odvontoges from o person or entity ofter the officiol votes, or otherwise tokes off iciol oction, to oword o 'public benef it" to thot person or entity. The prohibition opplies within ond outside of the geogrophicol boundories of Sonto Monico.
All persons or entities opplying or receiving public benefits from the City of Sonto Monico sholl provide the nomes of trustees, directors, portners, ond officers, ond nomes of persons with more thon o 10% equity, porticipotion or revenue interest. An exception exists for persons serving in those copocities os volunteers, without compensotion, for orgonizotions exempt from income toxes under Section 501(c)(3), (a), or (6), of the Internol Revenue Code. However, this exception does not opply if the orgonizotion is o politicol committee or controls politicol committees.
Exomples of o "public benefit" include public controcts to provide goods or services worth more thon $25,000 or o lond use opprovol worth more thon $25,000 over o 12- month period.

Thot informotion includes the nome of ony person or persons who is seeking o 'public benefit." lf the "public benefit" is sought by on entity, rother thon on individuol person, the informotion includes the nomeof everyperson who is: (o) trustee, (b) director, (c) portner, (d) officer, or hos (e) more thon o ten percent interest in the entity. Therefore, if you ore seeking o "public benefit" covered by the Ooks Initiotive, you must supply thot informotion on the Ooks Initiotive Disclosure Form. This informotion must be updoted ond supplied every 12 months.

1. Personol services controcts in excess of $25,000 over ony 12-month period;
2. Sole of moteriol, equipment or supplies to the City in excess of $25,000 over o 12- month period;
3. Purchose, sole or leose of reol property to or from the City in excess of $25,000 over o 12- month period;
4. Non-competitive fronchise owords with gross revenue of $50,000 or more inony 12-month period;
5. Lond use vorionce, speciol use permit, or other exception to on estoblished lond use plon, where the decision hos o volue in excess of $25,000;
6. Tox "obotement, exception, or benefit' of o volue in excess of $5,000 in ony 12- month period; or
7. Poyment of 'cosh or specie" of o net volue to the recipient of $10,000 in ony 12- month oeriod.
