Change Order 4 – South San Francisco
Speridian Technologies, LLC Page: 1

CHANGE ORDER FORM
This Change Order No. 004 (“Change Order”), effective on November 1, 2026 (“Effective Date”), is made to South San Francisco Services Agreement dated December 3, 2024 (“SOW”) between City of South San Francisco (“Customer”) and Speridian Technologies, LLC (“Provider”). This Change Order shall be governed exclusively by the SOW and this Agreement.
• Describe services or changes requested and the reason for the change (attach additional pages if necessary).

Speridian has implemented the Clariti software platform for the City of South San Francisco in accordance with the requirements documented in the approved Functional Design Documents (FDDs), Business Requirements Document (BRD), and Change Order No. 003. The system went live in September 2026, and Speridian has been providing Hypercare support to stabilize the system and support City staff through the transition.

Hypercare Progress Summary
Hypercare began on September 14, 2026. As of October 5, 2026, City staff have reported 210 Hypercare items, and 149 (71%) have been resolved (status of Ready for Prod, Closed, Completed, Duplicate, or Invalid).

Type Total Resolved Open Possible Move to
Parking Lot Being Worked
Bug 78 67 11 0 11
User Story 132 82 50 34 16
Total 210 149 61 34 27
New items reported have tapered off each week, an indication that the system is stabilizing:

Hypercare Week Dates New Items Reported
Week 1 September 14 – 20 125
Week 2 September 21 – 27 42
Week 3 September 28 – October 4 34
Week 4 (in progress) October 5 – 11 9 (as of October 5) Key Hypercare results:
• 67 of 78 bugs (86%) have been resolved.
• Setting aside the 34 user stories identified as possible Parking Lot items, 85% of in-scope Hypercare items have been resolved (149 of 176).
• The most urgent items were handled first. As of the October 2 data extract, 64 of 79 Priority 1 items were resolved.
• All data conversion items and 94% of user training items have been resolved (October 2 data extract).
• Weekly new items fell from 125 in Week 1 to 34 in Week 3, a 73% reduction. • In addition to Hypercare items, Speridian has worked 14 Parking Lot items during the Hypercare period, 6 of which are built, tested, and ready for production.

Speridian Technologies, LLC Page: 2
• 5 items are currently blocked pending responses from the City.

Hypercare items have been addressed according to the following criteria: bugs; and user stories related to cleaning legacy data, updating mapping to address legacy data migration issues, helping users move records through their workflow to continue processing, account questions (merging accounts, changing account types, and record visibility), and blockers to daily operations.

Remaining Enhancement Backlog
Through UAT and Hypercare, the City has identified a backlog of enhancement requests that fall outside the original scope, the items approved in Change Order No. 003, and the Hypercare criteria above. This backlog consists of 99 items:
• Appendix A – Parking Lot Items (74 items): enhancement requests identified during User Acceptance Testing and deferred to the Parking Lot.
• Appendix B – Hypercare Items Moved to the Parking Lot (25 items): items reported during Hypercare that are new requirements or refinements of existing requirements rather than defects. Additional Hypercare candidates identified for the Parking Lot will be confirmed with the City and added to the backlog.

Because the City’s priorities will continue to evolve as staff gain experience with the system, Speridian proposes to address this backlog through month-to-month post go-live support rather than a fixed list of deliverables. A dedicated Speridian team will work the backlog in the priority order set by the City.

This Change Order does not commit to a specific number of items. The number of items completed will depend on the size and complexity of the items the City prioritizes.

These requested changes represent an expansion of the approved project scope and therefore require a formal change order to address the associated effort, schedule, and cost. • The impact of this Change Order on the SOW, including any impact on:

Statement of Work: These are the changes to the original Scope in the SOW Post Go-Live Enhancement Support (Month-to-Month) – Added • Speridian will provide a dedicated support team of approximately 525 hours per month to analyze, configure, develop, test, and deploy items from the enhancement backlog (Appendix A and Appendix B).
• The City may add, remove, or substitute backlog items at any time. New items will be sized by Speridian and placed in the backlog according to City priority. • Services include requirements clarification, configuration and development, quality assurance testing, deployment to production, and project management.

Prioritization and Delivery Approach
• The City will designate a Product Owner who is authorized to set backlog priorities on behalf of the City.

EXHIBIT B
Change Order 4 – South San Francisco
Speridian Technologies, LLC Page: 3
• Speridian and the City will hold a weekly prioritization meeting to review the backlog, confirm priorities, and select items for the next two-week sprint. • Items will be worked in the priority order set by the City. Speridian does not commit to completing a specific number of items within the support period. • Completed items will be deployed to production following City UAT approval, on a release schedule agreed with the City.
• Items that are blocked pending City decisions, data, or third-party input will be set aside, and the team will proceed with the next priority item.
• Speridian will provide a monthly status report showing items completed, items in progress, hours used, and the remaining backlog.

Support Team
• Project Manager, Business Analyst, Salesforce Architect, Developers, and QA Analyst, allocated as needed to deliver the City’s priorities within the monthly hours.

Assumptions and Exclusions
• Open Hypercare items that meet the Hypercare criteria will be completed by Speridian as part of Hypercare close-out and are not billed under this Change Order. • The HDL integration and Canam-Teller integration remain under Change Order No. 003 and are not included in this Change Order.
• New integrations, new modules, and Salesforce or Clariti licensing are not included. • The City will provide timely decisions, UAT testing, and approvals so that completed items can be deployed.

Schedule and Cost: Support will begin on October 15, 2026 and continue for five months, billed at $42,000 per month or $80 per hour, for a total not to exceed $210,000.
