THIS CHANGE ORDER FORM SHALL BE USED FOR ALL CHANGE ORDERS ASSOCIATED WITH THE WORK. NO ADDITIONS OR DELETIONS TO THIS FORM SHALL BE ALLOWED, EXCEPT WITH PERMISSION OF THE CITY OF THOUSAND OAKS.
The Contract is changed as follows:

1 Perform all work in connection with, sealing of the roof joints, additional drainage and grading improvements around the exterior of the reservoir and along the access path, cleaning and restoring the site, hydroseeding the impacted areas, and additional trucking for removal of all the trash & debris, on time and material basis for a total not-to-exceed amount of $58,965.
CHANGE ORDER TOTAL $58,965
Contractor agrees to furnish all labor, materials, equipment and services to perform the Work, modified as described herein, in accordance with the Contract Documents as revised by this Change Order.

In consideration of the change(s) described above, the Contract Price is adjusted as follows:
The original Contract Price was $ 400,000
Net change by previously authorized Change Orders $ 2,000,000 The Contract Price prior to this Change Order was $ 2,400,000 The Contract Price will be changed by this Change Order in the amount of $ 58,965 The new Contract Price including this Change Order will be $ 2,458,965 In consideration of the change(s) described above, the Contract Time is unchanged:
The Contract Time shall remain in compliance with Article 2 of the contract.
Contractor accepts the terms and conditions stated herein as full and final settlement of any and all claims arising from this Change Order.
