SUBJECT: Request to amend Consent Calendar Item 7G
Item 7G is an Amendment to an agreement with Opt Health Inc. The Amendment references the Scope of Work as Exhibit B; however, the Exhibit B was inadvertently not included with the Amendment. Exhibit B is attached here.

Membership Program
1. Membership Benefits. Each Opt Health Optimization Membership includes:
• Initial labs with 55+ biomarkers
• 60-minute physician consultation and personalized treatment plan • Up to 3 supplements and micronutrients and 2 prescriptions, shipped 4x per yeal • Specialized prescriptions included to address hormone imbalances, chronic disease management, fatigue, sleep and energy issues, weight loss, fertility, and more based on unique health profile and goals
• Follow-up labs with physician consultation 4x per year for health check-ins and plan adjustments
• Dynamic dashboard to track Performance Score and other metrics • Online chat access to your care team through the Opt app • Access to ordering medications, including peptides, hair loss medication, ED medication, and more on the Opt app (additional fees apply)
• Access to epigenetic tests and cardiac, cancer, brain, nutritional, and toxin tests (additional fees apply)
2. Program Fees.
a. Intake Fee: $97.50
b. Monthly Membership Fee: $171.50
c. Cohort: 45 Memberships
3. Terms.
a. Department will purchase the Intake Fee and Monthly Membership Fees for 6 months for the Cohort prior to commencement of the Memberships
b. Total Program Fees are creditable and transferable (e.g., if Officer #45 uses a Membership for 2 months and cancels, a new officer, Officer #46, may use the remaining 4 months on Officer #45's Membership. If a Membership is being transferred, Department shall elect if the Intake Fee will be deducted from the value of a Monthly Membership, paid by Department, or paid by the new member.
c. Opt will provide reporting on the 5th of each month as to those members who have opted to cancel their membership
d. All Members will be subject to and governed by Opt's standard membership agreement documents that are agreed upon by each Member during the onboarding process.
