Imelda Huerta, Assistant City Manager
John Conley, City Manager Agenda Location: Discussion
SUBJECT: FIRST AMENDMENT TO SAN DIEGO RESCUE MISSION, INC. AGREEMENT FOR

CITYWIDE HOMELESS OUTREACH SERVICES
RECOMMENDATION: Adopt City Council Resolution No. 2025- , approving a First Amendment to the agreement with San Diego Rescue Mission, Inc., to provide citywide homeless outreach services.

CORRELATION TO CITY COUNCIL GOALS: Homelessness & Housing.

PRIOR ACTION: On June 10, 2025, adopted City Council Resolution No. 2025-99, approving an agreement with San Diego Rescue Mission, Inc., to provide homelessness outreach, housing navigation, and housing stability services (Vote: 5-0).

On March 25, 2025, City Council received an update on the San Diego Rescue Mission’s accomplishments during the first four months of contracted services and directed staff to continue the contract for citywide homeless outreach services (Vote: No vote taken).

On March 11, 2025, adopted City Council Resolution No. 2025-26, accepting Encampment Resolution Funding, Round 3, Grant Funds form the California Department of Housing and Community Development (Vote: 5-0).

On October 22, 2024, adopted City Council Resolution No. 2024-172, approving an agreement with San Diego Rescue Mission to provide citywide homeless outreach services. (Vote 4-1. Contreras opposed).

STATEMENT ON THE SUBJECT: On August 12, 2024, the City published a Request for Proposals (RFP) to solicit a provider for citywide homeless outreach services that included proactive street-based case management and coordination of citywide case conferencing using a By-Name List (BNL). Three proposals were received, evaluated by City staff and a representative of the Regional Task Force on Homelessness (RTFH), and on October 22, 2024, City Council approved an agreement with SDRM to provide citywide homeless outreach services. As part of this action, the City Council directed staff to conduct a four-month review of SDRM’s work for subsequent Council consideration.

San Diego Rescue Mission (SDRM) provides housing assessments to all individuals engaged, with entry to a Coordinated Entry System for possible matches to either Permanent Supportive Housing or Rapid Rehousing. For clients with resources to move directly into housing, SDRM facilitates the transition from the street into housing.

SUBJECT: FIRST AMENDMENT TO SAN DIEGO RESCUE MISSION, INC. AGREEMENT FOR

CITYWIDE HOMELESS OUTREACH SERVICES
SDRM provides weekly and monthly reports detailing outcomes, including:
•Number of unduplicated individuals served
•Exits from street homelessness
•Exits to permanent housing
•Individuals maintaining housing after 12 months of housing placement •Demographics of those being served with a focus on diversity, equity, and inclusion •Number of people served through shower trailer events
•Type of service interactions
•Progress on the BNL
•Success stories
•Community engagement activities
On March 25, 2025, the City Council received SDRM’s four-month review, which highlighted:
•SDRM’s use of the Shelter Ready app to facilitate referrals to homelessness shelters and expand access to shelters previously unavailable to Vista HOT, •the development and use of a By-Name List (BNL) to collect and report data on Vista’s unsheltered homelessness, and
•the implementation of a dedicated phone line (760-639-6124) and email address ([email protected]) for any interested party to make referrals for outreach services.

Based on SDRM’s performance and accomplishments presented in the four-month review, the City Council directed staff to continue the contract for citywide homeless outreach services with SDRM.

In June 2025, Council approved a separate agreement with SDRM funded by Encampment Resolution Funding Round 3 (ERF3) to provide outreach, housing navigation, and stabilization services. The citywide agreement works in coordination with the ERF3 program to operate as a singular Vista Homeless Outreach Team (HOT), ensuring consistency and alignment of services. SDRM provides housing assessments, case management, and coordination with regional partners, along with monthly reports on outcomes, demographics, and community engagement.

With ERF3 now serving as the City’s primary funding source for Vista HOT and SDRM, this amendment will extend the term of the Citywide outreach contract by eight months to June 30, 2026, to better align with current program needs. Staff recommends approval of the First Amendment to the agreement with San Diego Rescue Mission, Inc. (SDRM) for citywide homeless outreach services.
