RESOLUTION NO. 2026-XX

A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF FREMONT

AUTHORIZING THE CITY MANAGER TO EXECUTE A

MEMORANDUM OF UNDERSTANDING WITH ALAMEDA COUNTY

HEALTH TO ENABLE THE CITY TO RECEIVE REIMBURSEMENT
FOR ELIGIBLE FY 2025/26 EXPENSES THROUGH ALAMEDA
COUNTY’S TARGETED CASE MANAGEMENT (TCM) PROGRAM AND

DELEGATING THE CITY MANAGER OR FINANCE DIRECTOR THE

AUTHORITY TO SIGN, CERTIFY OR EXECUTE DOCUMENTS ON

BEHALF OF THE CITY WITH RESPECT TO THE TCM PROGRAM AND

MAKING FINDINGS OF CEQA EXEMPTION

WHEREAS, the City of Fremont (“City”) is a Medi-Cal program provider in partnership with Alameda County Health; and

WHEREAS, the City’s Human Services Department also participates in the Targeted Case Management (“TCM”) program; and

WHEREAS, the City is designated as a Local Public Entity (“LPE”) that allows it to self-certify its own public expenditures; and

WHEREAS, the City is required to sign, certify and execute documents pertaining to the TCM program.

NOW, THEREFORE, THE CITY COUNCIL OF THE CITY OF FREMONT RESOLVES AS FOLLOWS:

SECTION 1. The City Council authorizes the City Manager or designee to execute Memorandum of Understanding with the Alameda County Health to enable the City’s Human Services Department to receive reimbursement for eligible FY 2025/26 expenses through the TCM program.

SECTION 2. The City Council delegates the City Manager, Finance Director, or their designees, the authority to sign, certify or execute documents on behalf of the City, with respect to the TCM program.

SECTION 3. The City Council finds that this action is exempt from and not subject to review under the California Environmental Quality Act (“CEQA”) because, without limitation, it is not a “project” as defined in CEQA Guidelines Sections 15378(b)(2), 15378(b)(2) and/or 15378(b)(5), and it has no possibility of a significant effect on the environment, as specified in CEQA Guidelines Section 15061(b)(3), each as a separate and independent basis for CEQA compliance.
