“emergency” food and the system designed was largely transactional, with a focus on serving as many people as possible as quickly as possible.
After 30+ years as a Church charity RCFL became an official 501C3 nonprofit in 2020. In 2021, after one year as a freestanding non-profit the City of Rancho Cordova conducted a 90-day project to support RCFL to better understand its growth and capacity needs and to help identify future impact priority areas for our then fledging organization. The project findings made several recommendations. Hire an Executive Director, develop a strategic facility action plan, implement Sacramento Food Bank and Family Services “Oasis” data collection system, improve financial management and fund development, diversify the volunteer pool, introduce a stronger whole-person care approach to the program, connecting folks with a full range of services, become an official food recovery organization and concentrate on more nutritionally based foods.
In 2022, with the feasibility study as the road map, we began the initial steps to transform our organization with a focus on dignity, health, social justice, community, and person-centered design. We have reconfigured, repaired, brought up to code and food safety standards all buildings and equipment at our current location. Our food distribution operations have been designed to meet the growing demand while prioritizing food, guests, staff, and volunteer safety. Since 2022 we have seen a 400% increase in the number of households we serve. We received the Sacramento Food Bank and Family Services only 100% satisfaction rating for food, program, and safety compliance of any partner agency in the network.
In October of 2022 we invested in an IT infrastructure to launch the “Oasis” data collection system. Trained staff and volunteers can now accurately log our guests into the system, giving us accurate data on who we serve, the frequency and types of the services provided. We now meet reporting requirements that qualify us for program expansion. The data collected also gives us the ability to seek grants that require demographic data on who we serve.
A strong financial management system was put into place, including contracting with a part-time controller, and implementing internal financial controls. We completed our first financial audit for fiscal year 21/22 and are about to complete the audit for fiscal 22/23. These strong financial systems give us the ability to apply and qualify for a broader array of funding, including large grants. Compliant HR practices were implemented, and staffing was expanded. With our financials in order we focused on fund development, increasing the amount of funds raised significantly.
The growing demand for services required a broader and larger volunteer base. We have seen our volunteer team grow 150% over the last year. We now welcome a steady stream of current and former guests, City residents (of all ages), corporate groups, Church groups, students, parents and their children with disabilities, adults who are Neuro diverse and participants in the Sheriff Departments Alternative Sentencing program. Our original board of trustees has expanded 66% and now includes representatives from Solidigm, Dignity Health, the City of Rancho Cordova, local Corporations, and the Diocese.
We have focused on four primary areas, service with dignity, food access, food recovery and program partnerships.
Our day begins with our Morning Meeting, which includes all staff and volunteers. We open with words of inspiration and gratitude for our work and a deep recognition of who we are about to serve. Everyone is encouraged to greet our guests with smiles, providing a welcoming environment for folks who are struggling.
In addition to our three hour, three times per week distribution we have expanded distribution channels through community outreach and partnerships. Twice a month volunteers deliver groceries to a local senior apartment complex. Representatives from the Veterans Village weekly pick up groceries for veterans who need food.
Members of the new Community Schools team daily pick up groceries for families in transition. Healthy snacks are delivered by the pallet to Cordova High School monthly. Friday groceries are provided to over 23 families whose children participate in the Youth Center programs, the City’s Homeless Outreach team weekly picks up ready to eat ITEM 3.1.

food for the unhoused and groceries for the Transitional home, and in partnership with HART, we provided meals for the Winter Shelter.
Our ability to provide more fresh healthy food was bolstered by our designation as a food recovery organization.
Collecting edible food that would otherwise go to waste and redistributing it to feed people in need. With funding from last year’s CEIF grant we increased food recovery significantly, expanding the number of grocery stores in the program and the amount of food recovered. This has brought over 1.5 million pounds of food to those we serve, diminishing food waste.
Through community outreach and an expanded service model we have been able to acquire additional services for the folks we serve. Last year we qualified for two significant programs from the Sacramento Food Bank and the Department of Health and Human Services, the Diaper Program and Senior Program. We are now a “diaper depot” proving families with young children a monthly supply of diapers. And our new Senior Box program provides enrolled seniors with a monthly addition of shelf-stable, diet healthy food. Community partnerships with Code Enforcement have brought the Homeless Navigators onto our campus weekly to assist the unhoused. And a partnership with Family Meal Sacramento brings 200 restaurant prepared meals (each meal serves a family of four) to the food locker. We primarily distribute these meals to veterans, seniors, and students in the Community Schools program.
The last two years’ successes demonstrate our ability to meet the demands of the growing impact of food insecurity in our region.

Single Mom Strong: EmpowerME Preschool & Childcare, Rancho Cordova Provide a
Project/Program
Synopsis
This project includes the leasing, tenant improvements, licensing, furnishing, staffing, training and initial enrollment of an EmpowerME Preschool & Childcare location in Rancho Cordova. It is intended to house one infant, two toddler and two preschool classrooms, providing full-day preschool/childcare services for approximately 60-65 children.

Project/Program
Description
Funds will be used to open an EmpowerME Preschool & Childcare location in Rancho Cordova, providing full-day, high-quality, low-cost childcare services for approximately 62 children in one infant, two toddler and two preschool classrooms.
Our region is grappling with a severe childcare crisis, with single-parent households bearing the brunt. The statistics are staggering: according to California Childcare Resource & Referral Network, in 2019, there were 173,203 children with working parents in need of childcare in Sacramento County, but only 45,033 childcare spaces available. The pandemic only exacerbated this issue, with 156 childcare sites closing in Sacramento County (as reported by CBS News in 2021). Then, staffing shortages further strained this system, expanding the extreme lack of capacity. Even before the current supply issue, quality childcare was financially unfeasible to most single-income households. The pandemic and inflation only exacerbated those concerns. This crisis deeply impacts our community, our economy, and, disproportionately, single mothers.
To end this crisis, we must ensure equitable access to quality childcare and location is a key factor. According to bestneighborhoods.org, 11.39% of households in Rancho Cordova have a single-parent, with a high concentration of these homes in the areas adjacent to Highway 50, making this corridor an ideal target location for EmpowerME to best serve Rancho Cordova families and workforce. EmpowerME has been operating in Sacramento for nearly two years, and in Citrus Heights for four years, (being named “Best of Citrus Heights” in the “Preschool/Childcare” category for the past three!) We are confident we can replicate our success and make a difference in the lives of Rancho Cordova families.
Beyond providing critical childcare capacity, many unique factors of EmpowerME will make it an asset to the City of Rancho Cordova. From low student-teacher ratios, to trauma-informed care providers, and significant emphasis on social-emotional development, EmpowerME’s design challenges the status quo in many ways. We tackle the issue of affordability with our innovative nonprofit model and strong public and private partnerships.
State programs intended to reduce childcare costs for single or low-income working parents are severely flawed, and create ceilings for earning potential and economic mobility, perpetuating cycles of poverty. Limited, free childcare (Headstart) is available only to those with very low income: a parent of two children qualifies with a maximum annual income of $24,860.
CEIF APP #112
ITEM 3.1. ATTACHMENT 1
As individuals rise from minimum wages toward a more livable wage, they may qualify for childcare subsidies, which pay for a portion of childcare costs for households exceeding the Headstart threshold, and earning up to $78,120 per year in Sacramento County. There is currently a three-year waiting list for this program, and, if received, the benefits decline as income increases.
We empower single-parents and disrupt this broken system. EmpowerME provides high-quality early childhood education, and full-day childcare for school-age children during school closures, at 20-40% lower cost than regional average (savings vary per program). While all children are welcome at EmpowerME, and all families benefit from the reduced tuition, programs have been specifically formulated to meet the needs of singleparent families, with single-parents having the opportunity to volunteer in our centers for additional tuition discounts. Single moms can provide for their families, with the assurance that their children are being well cared for, and the significantly reduced financial burden greatly impacts their single-income family budget. Finally, Single Mom Strong Career Advancement programming supports single mothers in many other impactful ways while they pursue career goals.
Our request is for (up to) two-year funding, with the first year’s funds to be used to lease, build out, license and furnish the center, and the second to fund its opening, including initial marketing and associated costs, classroom supplies and materials, and funding the budget deficit for the first six months, until the project is self-sustaining (equal to approximately half of wages during that time). This one-time investment serves as seed funding creating permanent enrollment capacity for over 60 children. This method has proven successful in the past, as it was used to fund the initial construction of our Citrus Heights center, and its expansion three years later. In both instances, all goals were met without additional funding, and the program is self-sustaining.
In addition to providing quality, affordable childcare for at least sixty children at a time, these spaces in EmpowerME serve as a gateway for single mothers into all SMS programming and the associated family wellness. When under or unemployed moms can rely on quality, affordable childcare and pursue career goals, compounding positive outcomes are created that can never be fully quantified.

Goals
The goal of this project is to not only open an EmpowerME Preschool & Childcare Center in Rancho Cordova, but to ensure the program is selfsustaining by the end of the grant period, while maintaining the high-quality care EmpowerME is known for.
Within the first year, success will be measured by completion of leasing, construction, licensing and furnishing of the childcare center. Upon completion, staffing, marketing and enrollment will be the focus of the next six months of the project period. Despite our significantly lower tuition, break-even is reached with just 70% enrollment of center capacity. As outlined on the attached start-up budget, we expect to achieve this level of enrollment within six months, as outlined below:
Six-month enrollment goal:
Infant program: 66%
Toddler program: 50%
Early preschool program: 92%
Preschool Program: 67%
Minimum enrollment within six months: 39 students. This extremely conservative assumption allows more than enough time for hiring, training and staggered start dates of students to ensure staff and student success.
Enrollment will then continue to fill remaining capacity, without financial support required.
The second portion of this project will be considered complete upon achieving 70% enrollment. Success will be measured by our ability to achieve this milestone and become self-sustaining, while also maintaining our quality standards.
According to our 2021 survey of parents of EmpowerME participants, 100% of respondents are very satisfied with the level of care their child(ren) receive at EmpowerME. Additionally, 97% of respondents agree that their child is embracing the values presented at EmpowerME (compassion, honesty, love, gratitude, respect, responsibility, perseverance) and 100% of respondents agree that their child is better at naming their emotions since participating in EmpowerME. This data will be gathered annually to measure the effectiveness EmpowerME programs and services, via a participant survey.

The greatest foreseeable challenges are relative to leasing: finding a suitable space and/or zoning issues. Rancho Cordova Community Enhancement & Investment Fund Grant Selection Committee To Whom It May Concern:
I am writing to extend my support of Single Mom Strong. We believe very strongly in their mission of empowering single mothers and their children and have witnessed the tremendous impact they have made on the local community.
Single Mom Strong’s wrap-around services support the single-parent family in a variety of impactful ways.
From fostering connection to providing resources, support, mentoring and career coaching, single mothers in our region are fully supported by SMS. Additionally, their high-quality, low-cost preschool and childcare services not only allow these mothers to comfortably pursue career goals, but they also improve outcomes for their children by providing a solid foundation to their education. With so many children in our region being raised by single parents, and the massive reduction in childcare capacity that resulted from the pandemic, their expansion is crucial to our region's recovery.

Re: Single Mom Strong grant application for EmpowerMe Preschool in Rancho Cordova To Whom It May Concern:
It is my honor and pleasure to offer my full support and endorsement for Single Mom Strong to receive grant funding in the effort to add an EmpowerME Preschool and Childcare Center in Rancho Cordova, CA. This is a growing region that is in desperate need to have affordable accessible childcare, for single parents and Single Mon Strong is the perfect organization to tackle this.
I am a huge fan of Single Mom Strong and the services they provide to an underserved community: quality resources and solutions to address the challenges that disproportionately affect low-income women and children. Many may not recognize or may take these needs for granted, but Single Mom Strong provides comprehensive services to support these families through the most critical and vulnerable period of their lives.
The program is multi-faceted, and allows women to retain their dignity by offering exceptional value for an outstanding child care experience. As a working mother myself, I know I would not be effective in any part of my life if I didn’t feel as if my children were in a safe, well-cared for, reliable, educational environment while I worked. Single Mom Strong delivers in all of these categories, especially in light of the lack of affordable options in our region.
They also provide career support, through mentorships, and job skills training. Many of these women don’t normally have access to business professionals through our region, but Tara Taylor is a highly respected leader in the region and has connected many of these women with people like me to provide coaching, guidance, resume reviews and interview practice.
I have personally witnessed the love and support that is poured into these families through Single Mom Strong. When I heard about their annual Valentine’s Day event, where the Single Mom community comes together to honor to celebrate these women, I along with two of my daughters offered to volunteer. It was amazing to see the faces and joy these women got to experience together on a night where they may have been overlooked, in other years.

We helped with the children’s activities. They had their own pizza party, along with arts and crafts, along with an opportunity to learn martial arts. It was so much fun! It was well worth our time.

Single Mom Strong- Organizaon Informaon
Single-parent families are an underserved, at-risk populaon, who, without support, may experience disastrous health and socioeconomic outcomes that can affect generaons and balloon already burdened social services systems. Single Mom Strong's innovave blueprint deploys comprehensive resources like support systems, career advancement tools and extraordinary childcare soluons to improve these families' trajectories.
Single Mom Strong (SMS) is a local nonprofit organizaon with a mission to empower single mothers and their children through enrichment, encouragement and educaonal programs. According to “Be Healthy Sacramento”, 26% of children under the age of 18 in Sacramento County reside in single parent households. The BHS website states, “Adults and children in single-parent households are at a higher risk for adverse health effects. They are more likely to develop depression, smoke, and abuse alcohol and other substances… [They] suffer from lower perceived health and higher risk of mortality.” Addionally, children of single parents are more likely to have educaonal deficiencies, engage in high-risk sexual behavior, have lower occupaonal status and income as adults, and give birth outside of marriage themselves.
SMS works to prevent these outcomes for our families by:
-Fostering connecon: The community aspect of SMS is one of the most important to these mothers, who have litle to no support and find comfort in bonds built on their common experience. -Increasing economic stability: Career Advancement tools, like coaching and the Career Clothing Closet, directly influence career success.
-Proving childcare soluons: EmpowerME Preschool & Childcare is quality early childhood educaon, that's affordable, even on a single-income budget. Full-day childcare is also offered to school-age children during school closures.

Our meals are nutritionally balanced, vetted by our Registered Dietician to meet one third of the DRI for seniors. According to Meals on Wheels California, 18.2% of our seniors live alone. Our drivers provide a smile, a friendly visit and a status check to ensure all is well with our senior participants. Meals on Wheels America says that meals for a senior for one year costs less than a day in the hospital or a month in a care facility. Our service and nutritional support helps keep our seniors healthy, which prevents unnecessary hospital visits and costly premature institutilization with our nutritious meals, status checks and friendly visits that help keep the lonliness at bay. We provide nutrition education four times per year on topics relevant to our participants and helps them make better nutritional choices that better support their health and well being.
Meals on Wheels by ACC has been the senior nutrition provider since 2010, when the County of Sacramento could no longer afford the senior nutrition program. Over the past nearly fourteen years, we have streamlined our operations and worked to grow sustainably. The Pandemic uncovered a deep need in our community for nutritional support amongst our seniors and we are committed to find ways to continue to grow so we can serve more seniors in our community. This grant and continued support from the CDBG grant from the City of Rancho Cordova helps us do just that-- serve more seniors in the City of Rancho Cordova.

Goals
Serve 12,000 meals to eligible, homebound and frail seniors living in the City of Rancho Cordova. We anticipate 80 unduplicated seniors will be served under this grant. Meals are delivered frozen once a week in a box of five frozen meals, or a hot meal delivered daily Monday through Friday, 250 service days per year. Meals are made by a local caterer, Brick House Restaurant and Lounge, located in Elk Grove and brought to our Harlin Drive distribution center, where volunteers pick up their route items for delivery to the participants in Rancho Cordova. All of our meals meet one third the daily nutritional requirements for seniors and are approved by our Registered Dietician.
If awarded this grant, this funding will allow 12,000 more meals to be served to Rancho Cordova seniors than the CDBG funding alone can support- a grand total of 18,250 meals will be served.
Meals and participants are totaled from each daily or weekly routesheet and entered into our database for compilation. We report only meals served to Rancho Cordova seniors, and will provide the attached spreadsheet with quarterly reports to update you on our progress.

Meals on Wheels is primarily funded through Older Americans Act funding through the California Dept. of Aging and Agency on Aging/Area4. Meals on Wheels also receives support from Sacramento Housing and Redevelopment Agency, who executes CDBG funding for the City and County of Sacramento, CDBG and GF from the City of Citrus Heights, CDBG and CSG from the City of Elk Grove and CDBG Funding from the City of Rancho Cordova. We are actively working to increase our private grants, public donations and foundation grants so we can increase the number of meals and seniors we serve.

Meals on Wheels by ACC is the Senior Nutrition Provider and has operated the Senior Nutrition Program since 2010, when the County of Sacramento was no longer able to sustain the program. Entering our fourteenth year of operations, our programmatic goal is to continue to increase the number of meals and seniors we serve so no senior goes hungry in Sacramento County. We need the support of the public, our local governments and officials to achieve that goal. We are asking for $48,000, or 12,000 meals at $4 per meal to bridge the gap of what the CDBG grant is not able to fund and cover more of the meals MoW serves in Rancho Cordova. We are asking the same share of cost of $4 per meal of all the Cities and SHRA.
We are asking this year for a single year's funding as we hope to grow- to find and open a Community Cafe in Rancho Cordova and increase our program to mobile seniors in Rancho Cordova. The opening of a cafe would increase the future number of meals served to Rancho Cordova residents significantly and we will likely ask for additional support when that happens.

Meals on Wheels serves meals Monday through Friday, 250 days per year, excluding holidays:
Independence Day, Labor Day, Thanksgiving and the Day after Thanksgiving, Christmas Day, New Year's Day, MLK, Jr Day, President's Day and Memorial Day.
However, our home delivered meal participants receive a "double delivery" when a holiday falls on their normal delivery day- our hot participants receive one extra meal to warm up on the holiday and our frozen participants receive a second box of five frozen meals to cover the week the holiday falls so participants are not without their meal(s) due to a holiday.
Adaptability: What is
your plan to complete
your project/program
should any
challenges arise?
Meals on Wheels has a wait list of seniors waiting to start meal service and the City of Rancho Cordova is no exception. If we are falling short of meeting our service goals, we will start additional volunteer routes in Rancho Cordova to increase the number of meals served and seniors served to meet our goals.
Meals on Wheels delivers twice per year a five meal pack of shelf stable meals for each home delivered participant to keep in case of emergencies.
Those packs are funded by the County of Sacramento's ToT Grant. If such an emergency occurs where MoW is unable to deliver meals to a participant, we ask them to eat the meals in their SSM pack and we replace it during our next regularly scheduled delivery.
During the pandemic, Meals on Wheels continued to provide meals to our community's seniors. While we shut down our cafes, we did continue to deliver meals. We practiced safe delivery, masked up and stepped back to maintain social distance during the delivery, but made sure we saw each senior to ensure their well being. We are confident we can continue our service if there are other public health orders again.

Project/Program
Synopsis
Sierra Service Project will hire a team of 7 young adults for our second Rancho Cordova Home Repair Workforce Development Academy.
They will learn how to use power tools and complete home repairs for lowincome residents, develop leadership skills by hosting youth volunteers, and explore career opportunities through weekly speakers.

Project/Program
Description
From 2015 to 2023, Sierra Service Project (SSP) hosted 2,248 volunteer days and completed 118 exterior home repair projects for 97 lowincome City residents. These projects primarily took place during the school year, with volunteers joining us for a day, weekend, or week of service.
During the summer of 2023, the City funded our first-ever summer program in Sacramento County. With the City's support -- and thanks to the hard work of 67 overnight volunteers and 36 local Rancho Cordova youth -- SSP completed 12 home repair projects last summer, including our first ever wheelchair lift. Attached is a selection of personal reflections from volunteers and staff who participated in our 2023 summer program in Rancho Cordova.
In March 2023, SSP participated in the first round of the City of Sacramento's Climate Action and Resiliency Youth and Young Adult Pathways Program. Our program, the Sacramento Sustainability Academy (SSA), offers paid workforce development opportunities for youth and young adults. As of the time of this submission, 27 young people have graduated from this program, with three new cohorts planned for this spring and summer.
SSA is the inspiration for our current CEIF funded program, the Rancho Cordova Home Repair Academy (Academy), that runs from March 26 to May 18, 2024. Building on our history of providing youth and young adults with hands-on experience with tools, home repairs, and high impact community projects -- and our recent success equipping young people with valuable job skills and hands-on experiences -- the Academy is focused on exterior home repair projects for low-income residents as well as workforce learning opportunities and connections.
Academy fellows will spend 24 hours a week learning and completing home repairs for low-income Rancho Cordova residents. Every week, a local leader will speak with the group, exposing them to new career opportunities. This team will build hands-on building techniques, gain CEIF APP #114

exposure to various construction trades and community service organizations, and learn and practice conflict resolution and leadership skills.
After building their skills, the cohort will host four Saturday Days of Service for local youth volunteers. They will practice their leadership skills by teaching and mentoring youth, while having extra hands to complete home repairs for low-income residents. Near peer leadership has always been a staple of our programs, and we are excited for the cohort to lead local youth in passing along their newly gained knowledge.
Since our current program will launch immediately after this application is due, our proposal this year is very similar. We anticipate adjusting our program proposal after we receive feedback from our first Rancho Cordova Home Repair Academy this spring. This proposal includes a two week longer prep period for our Program Manager to recruit Academy fellows and youth volunteers, a $1 per hour pay increase for all roles (aligned with the minimum wage increase), and a 5% increase for building materials, food, and other supplies to account for inflation. Stipends, a motivator for engaging youth, were added for 80 local youth volunteer days.
The Rancho Cordova Home Repair Academy benefits multiple constituent groups in Rancho Cordova: by preparing young adults entering the workforce to be confident in their skills, equipping teenagers looking for volunteer opportunities, assisting low-income residents who need help maintaining and repairing their homes, and businesses who are eager to connect with, mentor and employ young adults.
Tell us about your

Team
SSP will hire one Program Manager to lead the cohort directly, reporting to SSP's Executive Director, Megan Taylor, who has spent 14 years managing SSP’s operations. Together, they will develop the program in its second year, evaluating past challenges to implement changes.
The cohort will consist of seven Rancho Cordova residents (ages 18- 30) who will be paid Academy fellows. One more experienced fellow will be a supervisor, working with the Program Manager to hire, train, and lead the rest of the cohort. The cohort will complete home repairs for lowincome City residents, and lead local youth in Days of Service.

Goals
1. Complete 4 to 8 home repair project units between March and May 2025. These projects will improve the quality of life and safety of homes for residents, and community pride by repairing or replacing fences, porches, stairs, ramps, and exterior paint.
2. Academy fellows will learn new hands-on job skills, how to work as a team, leading youth volunteers, and explore additional career pathways. Fellow satisfaction survey responses will be over 80%.
We will send out additional surveys one year after the program has ended to begin to measure the impact of the workforce development in these young people and their employers.
3. Host local youth for 80 volunteer days to provide additional labor to complete requested home repairs, and near peer leadership opportunities for Academy fellows. Volunteer satisfaction survey responses will be over 80%.
4. Complete high quality home repairs which are a source of pride for homeowners, neighbors, the City of Rancho Cordova, Academy fellows, and youth volunteers. Homeowner satisfaction survey responses will be over 80%.

SMUD awarded SSP a Shine grant for $40,525 for 2024, which overlaps with our current CEIF grant and this proposal. This includes local youth volunteer stipends ($25 per day) and home energy auditing equipment and training. We plan to offer free home energy audits to all Rancho Cordova residents we work with, funded by this Shine grant. We will apply for more significant SMUD funding in July through our growing partnership and their career pathway opportunities.
SSP is also a sub grantee of the Coalition for Home Repair, which received $1,000,000 from the U.S. Department of Housing and Urban Development Veterans Home Rehabilitation and Modification Pilot program grant. We have $40,000 allocated to four Veteran's homes through 2026.
Eligible Veterans in Rancho Cordova can access this funding to supplement the project needs not covered by CEIF, including interior home safety improvements.
We just submitted a $50,000 proposal to Sacramento County's Transient Occupancy Tax (TOT) grant program to fund days and weekends of service in 2025 for home repairs and community based projects throughout the County. A 2023 TOT grant helped support youth stipends in Rancho Cordova and other expenses not covered by CEIF.

As a primarily outdoor based program during mild weather months, this program inherently reduces the risk of spreading respiratory illnesses. SSP has been very COVID cautious to protect the health and safety of vulnerable community members, volunteers, and staff, including regular testing in our workplace culture.
During inclement weather, we have partner organizations throughout Rancho Cordova that we can shift to do indoor work for (Fitrah, Rancho Cordova Food Locker, and The People's Pantry). We know these organizations often have special projects that need an infusion of one-time volunteers, and partnering with them helps Academy fellows learn about other nonprofits and resources within the community.
We have a team of committed weekly volunteers (retirees) who are able to fill in any project completion gaps, as they did a few years ago for us during the pandemic. They are also a great resource for our fellows to learn building skills.
While this schedule is planned for February through May, this program is flexible to shift during the year to be earlier if a Program Manager is identified sooner. We are cognizant of school schedule limitations for youth volunteers, and will aim to shift as able to better accommodate their availability.

Dear City of Rancho Cordova,
Hello! My name is Pravin Rajah. I’m a current senior at Lincoln High School in Stockton, CA.
I’ve been volunteering with the Sierra Service Project over the course of two years through leadership in the Youth Advisory Council, volunteering at their headquarters, and summer programs.
Initially, I was a volunteer at the headquarters but gradually I became more involved, going to my first summer program in the summer of 2022 in San Diego and another summer program in the summer of 2023 in Rancho Cordova. In addition to this, I’ve been volunteering on some weekends, known as days of service. Usually, these days contain smaller groups allowing me to become closer to the people I’m working with.
One such group, El Parkis, and I have become much closer due to this. Because they are based in the Sacramento and Elk Grove region, it’s been incredible working alongside them and fostering such a fun and interesting relationship with them through the few weekends of service I’ve done alongside them and the summer program of 2023. These sessions have not only benefited me socially. SSP places a large importance on addressing problems cities are facing today.
During my time at the summer program in 2023, we spent a large amount of time working at food banks and creating garden beds for community use. Addressing problems such as these through SSP while also learning future skills such as how to use construction tools has been extremely useful to me for use at home, and especially now during a time where it’s most crucial to have your voice heard.
Overall, SSP has had a tremendous impact on my life. I have loved working alongside former and current volunteers, building new friendships and fostering new connections within our communities.
Please feel free to contact me for more information.
Thank you for your time and consideration,
Pravin Rajah

Cut to the Chase
Cut to the Chase focuses on improving mental health as a social deterrrinant of health in Black communities by providing mental health resources to clients in barbershops and beauty salons. By removing barriers to mental health support, this project promotes Innovative service delivery and advance health equity.

Provide a Project/Program
Description
According to the U.S. Health and Human Services Office of Minority Health, adult African-Americans are 20 percent more likely to report serious psychological distress and more likely to have feelings of sadness, hopelessness, and worthlessness than their White counterparts. Our proposed project, Cut to the Chase, addresses this disparity by merging professional mental health support, peer support and stigma reduction efforts within the informal African-American gathering spaces of local barber shops and beauty salons in the Greater Sacramento area.
Since 1968, Greater Sacramento Urban League (GSUL) has assisted thousands of Blacks and other marginalized community members to gain meaningful employment, access safe, affordable and stable housing, reach their educational goals, overcome health disparities, and improve their overall health and wellness. GSUL programs and services are driven by a five-pillar empowerment agenda, which includes: civic engagement and leadership development; civil rights and racial justice; economic empowerment; education and youth; and health and quality of life.
Cut to the Chase features monthly group therapy sessions, stigma reduction efforts, and one-on-one therapy sessions at local barber shops and beauty salons to increase help seeking behaviors, decrease stigma, and increase knowledge of available resources for African-Americans. The team heading this effort, recognizes that social and community contexts as well as environmental conditions play major roles in health outcomes for Black communities. Additionally, stigma exists around discussing adverse childhood experiences, trauma, emotionality, and seeking help for mental-emotional health issues. Furthermore, the absence of social infrastructure, spaces, and opportunities which are safe and supportive for interconnection and interpersonal communication hinders the community’s ability to reduce stigma and address mentalemotional health proactively. In order to realize a positive impact on African American physical and mental health disparities, our project will address and measure the following short-term outcomes as the pathways to promoting health and reducing morbidity: decrease health-risk behaviors, improve quality of life, and increase productivity in life and longevity of life. To support mental-emotional and social health challenges within the Black community, this project will focus on using evidence-based programs to teach, train, and inspire Black leaders to incorporate interpersonal communication, social activity as well as trauma-informed care practices into their households, professional, and academic spaces. By providing culturally responsive mental health resources this project aims to create a community of mental health advocates who recognize the importance of community support services and resources.
With regards to the City of Rancho Cordova’s Community Enhancement & Investment Fund guidelines, this project specifically addresses increasing the quality of life for the people of Rancho Cordova with a secondary focus on business/economic development. The census identifies 10.9% of Rancho Cordova’s population as Black or African American alone with another 11.2% representing individuals who identify as two or more races. These are some of the target demographics that Cut to the Chase wants to service. In addition, the connections that Cut to the Chase fosters urges for a stronger sense of community and belonging.
Our program serves a secondary purpose of economic and business development through a partnership with a local hair salon and/or barbershop. We compensate for the location in order to reserve time in the evening to conduct these sessions. In the partnership, the business stays open to welcome clients. This environment creates a safe space for participants by presenting a low-stakes conversation in a familiar setting. It makes the group sessions feel much more like a round-table discussion rather than the actual therapy session. The mode of delivery for this service is through a contracted mental health professional and nurse. Barbers and hair stylists can choose to participate or even perform their skill and artwork all while receiving the free service. The influx of participants act as a form of business development as it attracts new customers to the shop both local and external. As a program currently administered in Sacramento, the program has garnered participants from outside of the area. In a way this showcases the host city to the visitor.

The purpose of the therapy sessions is to foster connections among participants. What makes this initiative remarkable is our utilization of spaces frequented by Black men and women. Should we need to adapt our approach, we would engage members through an online platform, employing innovative methods such as group painting, hair tutorials co-facilitated by therapists and beauticians, and yoga classes co-facilitated by therapists and beauticians. The primary objective remains equipping clients with mental health resources while providing a platform to alleviate stress. 4 - Project/Program Location

To Whom It May Concern:
I am writing this letter to express my enthusiastic support for the Greater Sacramento Urban League's mental health initiative, "Cut to the Chase," aimed at improving mental health outcomes for Black men and women hopefully launching in Rancho Cordova. This initiative is crucial in addressing the ongoing mental health disparities within our communities, particularly among Black individuals who face unique challenges and barriers to accessing mental health services.
It is imperative that we continue to advocate for and prioritize the mental well-being of Black individuals, and the approach taken by the "Cut to the Chase" program is both innovative and impactful.
By providing therapy in spaces where Black men and women already feel comfortable, such as local barbershops and beauty salons, the program effectively addresses the stigma surrounding mental health support within the community.
Having witnessed the success of “Cut to the Chase” in Del Paso Heights and Oak Park, I am confident in the ability of the Greater Sacramento Urban League to make a significant difference in the lives of individuals in Rancho Cordova. This initiative not only provides much-needed support but also fosters a sense of community and belonging, which is essential for healing and growth.
I wholeheartedly support the "Cut to the Chase" initiative and commend the Greater Sacramento Urban League for their commitment to addressing mental health disparities within the Black community. I am eager to see the positive impact this program will have on the lives of Black individuals in Rancho Cordova and beyond.
Sincerely, Monique Webb

(4) Upon making payment on any completed phase of the project, and before making any further payments, require your contractor to provide you with unconditional "Waiver and Release" forms signed by each material supplier, subcontractor, and laborer involved in that portion of the work for which payment was made. The statutory lien releases are set forth in exact language in Section 3262 of the Civil Code. Most stationery stores will sell the "Waiver and Release" forms if your contractor does not have them. The material suppliers, subcontractors, and laborers that you obtain releases from are those persons or entities who have filed preliminary notices with you. If you are not certain of the material suppliers, subcontractors, and laborers working on your project, you may obtain a list from your contractor. On projects involving improvements to a single-family residence or a duplex owned by the individuals, the persons signing these releases lose the right to file a mechanics' lien claim against your property. In other types of construction, this protection may still be important, but may not be as complete.
To protect yourself under this option, you must be certain that all material suppliers, subcontractors, and laborers have signed the "Waiver and Release" form. If a mechanics' lien has been filed against your property, it can only be voluntarily released by a recorded "Release of Mechanics' Lien" signed by the person or entity that filed the mechanics' lien against your property unless the lawsuit to enforce the lien was not timely filed. You should not make any final payments until any and all such liens are removed. You should consult an attorney if a lien is fil i t t "
Customer Signature _____________________________
Notice to Owner: "Under the California Mechanics' Lien Law, any contractor, subcontractor, laborer, supplier, or other person or entity who helps to improve your property, but is not paid for his or her work or supplies, has a right to place a lien on your home, land, or property where the work was performed and to sue you in court to obtain payment. This means that after a court hearing, your home, land, and property could be sold by a court officer and the proceeds of the sale used to satisfy what you owe. This can happen even if you have paid your contractor in full if the contractor's subcontractors, laborers, or suppliers remain unpaid. To preserve their rights to file a claim or lien against your property, certain claimants such as subcontractors or material suppliers are each required to provide you with a document called a "Preliminary Notice." Contractors and laborers who contract with owners directly do not have to provide such notice since you are aware of their existence as an owner. A preliminary notice is not a lien against your property. Its purpose is to notify you of persons or entities that may have a right to file a lien against your property if they are not paid. In order to perfect their lien rights, a contractor, subcontractor, supplier, or laborer must file a mechanics' lien with the county recorder which then becomes a recorded lien against your property. Generally, the maximum time allowed for filing a mechanics' lien against your property is 90 days after substantial completion of your project.
TO INSURE EXTRA PROTECTION FOR YOURSELF AND YOUR PROPERTY, YOU MAY WISH TO TAKE ONE OR MORE OF THE FOLLOWING STEPS:
Require that your contractor supply you with a payment and performance bond (not a license bond), which provides that the bonding company will either complete the project or pay damages up to the amount of the bond. This payment and performance bond as well as a copy of the construction contract should be filed with the county recorder for your further protection. The payment and performance bond will usually cost from 1 to 5 percent of the contract amount depending on the contractor's bonding ability. If a contractor cannot obtain such bonding, it Require that payments be made directly to subcontractors and material suppliers through a joint control. Funding services may be available, for a fee, in your area which will establish voucher or other means of payment to your contractor. These services may also provide you with lien waivers and other forms of protection. Any joint control agreement should include the addendum approved by the registrar.
Issue joint checks for payment, made out to both your contractor and subcontractors or material suppliers involved in the project. The joint checks should be made payable to the persons or entities which send preliminary notices to you. Those persons or entities have indicated that they may have lien rights on your property, therefore you need to protect yourself. This will help to insure that all persons due payment are CEIF APP #116

Home and Neighborhood Improvement Campaign
Provide a
Project/Program
Synopsis
This project will enhance the residents’ quality of life, community pride, and engagement. It will result in improvements at 30 low-income households each year. Homeowners are older adults, those with disabilities, or families with children. Homes will receive critical, health, safety, beautification upgrades and minor safety modifications.
Provide a
Project/Program
Description
This project will support Rebuilding Together’s commitment to improving homes and neighborhoods in the City. The two year home improvement campaign will enhance the residents’ quality of life, community pride, and engagement. This project is also aligned with the Housing Element’s goal to encourage the improvement, rehabilitation, and revitalization/reinvestment of the City’s existing residential neighborhoods.
This is a two-year request to improve 30 homes each year. Both years, 13-15 homes will receive extensive health, safety, efficiency, and beautification upgrades and 15-17 will receive minor safety modifications.
The work will vary per home but will include upgrades such as landscaping, fencing, painting, accessibility modifications, electrical, plumbing, carpentry, and energy efficiency. The work will occur throughout the year by volunteer groups, contractors, staff, students, Americorps members, etc.
The selection process includes community outreach, an application process, and a home preview using a safe and healthy homes assessment. The assessment is focused on identifying deficiencies in the homes related to the principles of keeping the home safe, dry, ventilated, pest-free, accessible, energy-efficient, and maintained.
We also identify exterior improvements to enhance the look of the home and neighborhood. The homeowner is also interviewed to identify any other issues such as food insecurity. Community and City resource information is shared with those in need of additional assistance. Tackling substandard housing in neighborhoods cannot be accomplished by one organization in isolation no matter the quality of services and resources invested. We leverage the strengths of volunteers and partners including nonprofit and faith-based organizations, local businesses, financial institutions, occupational therapists, and other service providers to accomplish the project goals.

We are requesting the grant for a two-year period of time to maximize the use of the funds and achieve the best results. It typically takes up to 3-5 months to ramp up before repairs can occur. By moving to a two-year project the startup time is eliminated the second year. We can select all the homes earlier to avoid starting over the next year. It allows for more time building relationships with the homeowners and partners, raising funds and recruiting volunteers. This approach is more attractive to funders because of the wider spread impact. We are confident in the ability to secure financial, human and in-kind resources due to 33 years of experience. The project is valid if it is only funded for one year.
Our Board of Directors and staff is committed to working in communities, such as Rancho Cordova, where there is an interest in neighborhood improvement and the residents’ well-being. Our close working relationship with Rancho Cordova staff has resulted in a positive experience for all and tremendous results. We plan to continue asking for funding as the need is growing, particularly among the low-income, older adult population who have limited housing options.

schedule
If we are awarded funding for two years, the project will begin with outreach in July 2024, followed by the application process. We expect home repairs will begin in early fall 2024. This will be continuous through the end of the contract.
Typical schedule would be:
Community outreach: July - September Outreach includes mailings, coordination with City staff, Code Enforcement, communication with local organizations, etc. We also consider homes that have participated in our other home improvement services in the City.
Application processing and home previews: September - December If appropriate applications are received, some homes will be selected in the Fall so work can begin early. The homes that receive minor home safety work only will occur beginning in August.
Scope of work development: A plan is created for each home based on need, labor, and budget. Some of the homes are selected for one-day projects and others with heavier scopes will occur over time, but be completed prior to June 30 .
Workdays: Volunteer workdays will be scheduled throughout the year depending on volunteer availability and the scope of work.
Skilled contractors will be scheduled as needed. (where licensed contractors are required to perform tasks)
Year Two: Outreach and home previews will occur during the first year to ensure that an adequate number of homes are selected for the next year.
The timeline in Year One will be repeated during Year Two, although work will begin in the Fall on those homes selected the prior year.
Adaptability: What is
your plan to complete
your project/program
should any
challenges arise?
We have been able to adapt our program delivery based on obstacles in the past (ie, funding delays, staff turnover, etc.) However, the public health orders during the pandemic provided a significant challenge for everyone.
We modified our model those years resulting in 100% CEIF grant compliance. We have integrated some of the adjustments and lessons learned into our current model which is flexible in case the orders change again.

Support Services to Homeless Veterans Program at Rancho Cordova Provide a
Project/Program
Synopsis
This grant will allow those who had been serving in our military and are homeless or at risk of homelessness an opportunity for housing, employment, and a bunch of comprehensive services. Thus, this project is to enhance the living conditions of 80 homeless veterans in the Veteran Program at Mather.
Provide a
Project/Program
Description
Veterans face a myriad of challenges, some of which are unique to veterans. Among the many issues facing veterans is homelessness.
Homelessness among US veterans is a serious and growing problem. The issue is exacerbated by a variety of risk factors that make veterans more vulnerable to homelessness than other civilians. Some of the identified factors are: COVID-19 pandemic, job loss, mental health problems, health problems, lack of health insurance, foreclosure, and family problems.
VOA's Veterans Program at Mather provides supportive services to lowincome veterans who are facing homelessness including housing, housing assistance, counseling, employment training, temporary financial assistance, case management, and other supportive services (e.g. access to food, household items, and furniture). The housing at the Mather Campus is basic studio units.VOA originally had 40 units but due to the increase in need, now offers 80 units to accommodate 80 single veterans.
VOA's Veteran Program at Mather currently offers 80 beds for both men and women for an average length of stay of 6 months as they work towards removing the barriers they face to allow them placement in permanent housing as rapidly or as appropriate for their needs and progress. The program offers temporary housing along with assisting clients with locating permanent housing, employment and other supportive services. While assisting homeless veterans with removing housing barriers and locating permanent housing, a large focus is heavily placed on increasing veteran’s employment potential through job readiness and retention training that will increase both earned and benefits income, allow them to return back to the community and sustain their independence. The case management services are designed to improve veteran families’ housing stability and may include assistance in securing housing, daily living needs, health care, transportation, fiduciary and payee services, legal aid, child care, financial literacy, etc. Additionally, we connect veterans to the various resources available through Veterans Affairs and other government programs.Wraparound Services Provided: -Temporary Transitional Housing -Balanced Meals -Case Management -Assistance with Accessing Benefits -Employment Services -Permanent housing search assistance -Support addressing housing barriers -Credit repair and financial literacy -Community resourcing and linkage.
Through this grant, VOA is planning to continue providing an abundance of financial and supportive services to each
veteran in the Program, including but not limited to the following activities:
CEIF APP #118
ITEM 3.1. ATTACHMENT 1
- Short-term financial assistance with rental and utility arrears; - Deposit and 1st month rent;
- Employment package (skilled workers tools, certification, resume writing, interview training, professional attire …etc);
- Job fairs & job placement assistance;
- Assistance with cost of training, equipment and other job-related needs; -Transportation services (bus passes, fixing vehicle or maintenance); - Provision of welcome home items and sanitation supplies; - Child care services.
While staffing and volunteer participation have decreased, the need for resources has increased because more veterans are homeless and rely on the help of VOA and generous foundations, and thus, unexpected expenses have been imposed on VOA's Veteran Program. Implementing health directives, applying precautionary measures, volunteers limited participation, inflation rising prices, and increased cost of living have all contributed to the funding shortage for 2024 fiscal year. Therefore, VOA has been considering targeted solutions to its Veteran Program to ensure that food-insecure and socially isolated veterans can stay fed, safe and healthy during the crisis.
Community Enhancement & Investment Fund would be of great help in providing support for the new units being opened to host more homeless veterans in the Rancho Cordova community.
Tell us about your
Project/Program
Team
VOA’s Veteran Program Admin Team is responsible for all administrative, programmatic and financial measurements. Goals outlined in this proposal will be evaluated by regularly auditing case files and tabulating results. Ms.
Koren Lamar, Veteran Program Manager, is responsible for the accuracy and completeness of case files and reports on outcomes regularly to funders and VOA's administrative office. A detailed status of each veteran is reported on a monthly basis along with services delivered. An important part of the grant implementation is working with the case workers, program managers and development officers to ensure that the outputs are SMART.

Adaptability: What is
your plan to complete
your project/program
should any
challenges arise?
Although the VA remains committed to the goal of ending veteran homelessness, no one agency can do it alone. VA and
federal, state, local partners, as well as non-governmental partners, for example: VOA, recognize that ending homelessness
is not a single event in time; rather, it is a deliberate effort made to achieve the goal, and continued follow-up efforts to make
sure that progress toward achieving the goal is maintained.
As an organization, VOA is regularly assessing the way we respond to the critical needs of our veterans. Funds to provide project support will continue to be sought through corporate and private foundations, as well as individual donors and corporate sponsorships. This funding will be used to leverage grant funding and large individual gifts from donors who have a passion for serving veterans and their families. With funding from CEIF, veterans will be helped achieve residential stability, increase skill levels and income, and obtain greater self-determination, and in the long run, they forever change the lives of hundreds of Guards and Reservists at risk of or experiencing homelessness in our community.
VOA has the expertise and trained staff to work under all circumstances.
VOA adapts flexible plans to implement its projects. Other arrangements may be rescheduled, organized and coordinated with CEIF during the grant term. However; all plans are dynamic at this point and are subject to further adjustments depending on the situations.

Notes: (Please add below any notes applicable to the funding of this grant) Past Client Support from The City of Rancho Cordova have encouraged local foundations to give towards VOA’s veterans related efforts. VOA will continue to utilize the support The City of Rancho Cordova provides as an example of charity investment in the lives of veterans experiencing homelessness. Definitely, this advocacy will allow VOA to attract other major gifts, foundation grants and corporate partners who will see this project as sustainable and motivate them to provide funding. VOA does not currently have a matching opportunity, but would use this funding to seek out a match for philanthropic support and long-term commitment.

MEMORANDUM FOR Julie Baumgartner, Director of Veteran Services, Volunteers of America SUBJECT: Grant Application
1. The California Military Department, and many of its Army and Air National Guard service members have been the beneficiary recipient of the services provided by Volunteers of America. Under the current voucher program, our service members have been eligible to receive emergency financial assistance to help out with short-term financial situations. Traditionally, these type of voucher programs have not been extended to National Guard members, however because of the success of this current program, we strongly support volunteers of America in its quest to secure more funds for this life-changing program.
2. In addition to the support the voucher program provides to the individual servicemember and their families, I will also add that it provides a huge morale boost to the organization and to the senior leaders in the organization, whose moral and ethical leadership outlook has been significantly improved, because of the ability to help servicemembers and their families, and in some cases save lives.
3. For any questions regarding this request, please contact the undersigned at [email protected].

"Revolutionizing Restaurant Revenue: The Power of Ghost Kitchens and Food Delivery Apps”
During the pandemic, a new concept emerged in the restaurant industry known as "ghost kitchens." These are commercial kitchens that lease space in their facilities to food businesses. Ghost kitchens enable individual businesses to safely prepare and sell their food through various food delivery apps like UberEats, DoorDash, TooGood2go, etc. We have found utilizing these apps is essential for financial success. Daily, we're able to generate between $1200 and $4000 in sales on each app. In a year participants will have earned close to enough to open their CEIF APP #119

EqualBite Restaurateur Incubator
Program Proposal
EqualBite Restaurateur Incubator Program Synopsis
EqualBite Restaurateur Incubator Program is a transformative program designed to uplift the Rancho Cordova community by fostering food-based entrepreneurship. Our step-by-step incubator equips under resourced individuals with essential skills and resources, guiding them through business development and capital building. Together, we create a path to quality restaurant ownership and enrich the community.
EqualBite Restaurateur Empowerment Incubator
Description
The EqualBite Restaurateur Empowerment Incubator Program endeavors to serve as a comprehensive resource for participants who aspire to become thriving restaurant owners in the city of Rancho Cordova. Our approach is twofold: fostering the emergence of new restaurant owners within the city while simultaneously equipping cohort participants with essential resources crucial for their success. As accomplished Rancho Cordova restaurateurs, we are thrilled to mentor cohort participants from inception to launch, assisting them in legitimizing their food-based businesses and acquiring the necessary capital to open their brick-and-mortar restaurants in Rancho Cordova.
The program name EqualBite embodies the enduring desire for a slice of the metaphorical “American Pie”, representing hearty opportunities and the sweet taste of success achieved by those resilient enough to forge a path from humble beginnings. In today's America, securing a bite of this symbolic pie has grown markedly challenging, hindered by factors like pandemics, rising inflation, and volatile housing markets. As a result, parts of our community face heightened levels of income inequality, poverty, homelessness, and a lack of resources for attaining financial stability which is why we aim for 40% of participants to come from income-insecure households.
The program's operation will entail:
1. Enrollment: We can admit 10 participants.
2. Recruitment: We will utilize social media, community boards like NEXTDOOR, Rancho Cordova Community programs, and local news channels to gather potential participants.
Workshops will begin July 1st and take place weekly in person and via Zoom Workshops will take place Monday, Thursday, Friday, Sunday 6:00pm- 8:00pm Workshop 1: Business Visioning I - Participants will explore their motivations ("Why"), create vision boards, envision the future of their businesses, and discuss strategies for building their core team.
Workshop 2: Business Basics I - Participants will establish their EIN and LLC.
Workshop 3: Food Handlers Certification - Participants will undergo modules, study, and take the food handlers exam.
Workshop 4: Food Managers Certification – Participants will study modules and prepare for the food managers exam.

Project Name: Innovate Rancho Summit: Navigating the Future of Rancho Cordova's Smart City Vision
Funding & Costs:
To enhance flexibility in fund allocation for the Innovate Rancho Summit, we propose consolidating the cost categories into broader segments. This approach allows for dynamic utilization of funds based on evolving program needs and priorities:
Program Delivery: $100,000
This includes expenses related to personnel (experts, moderators, operational staff), technology and equipment for interactive sessions (A/V needs, tech setup), and materials for the event (handouts, educational content). Consolidating these items under "Program Delivery" provides the flexibility to allocate funds where they're most needed, whether that's securing top-tier speakers, enhancing participant engagement tools, or developing high-quality program materials.
Event Operations: $50,000
This segment covers transportation logistics for speakers and team members, venue rental, catering, and any unforeseen or miscellaneous expenses. Grouping these costs allows for adaptive management of event logistics, ensuring the ability to negotiate the best rates for venue and catering services and to adjust transportation logistics as required.
Total Requested Funding: $150,000
This streamlined budget structure ensures that the Innovate Rancho Summit can adapt to changing circumstances and make the most effective use of funds to achieve the event's goals. It retains the focus on delivering a high-quality, impactful program while providing the operational flexibility needed for meticulous planning and execution.

Synopsis
The Jordan Project aims to start “The 200 Program” which plans to offer resources for the local homeless population suffering due to addiction, mental health issues, economic crisis, and trauma. Our goal is to help restore lives through mass outreach events, connection to food resources, shelters, and other needed resources.

Description
Background:
The Jordan Project "The 200 program" (The 200) is a program that is focused to serve the homeless population in Rancho Cordova on the frontline. (The 200) was developed by Joshua Domingos and inspired by his past. Mr. Domingos was homeless just 7 years ago living in the streets of Rancho Cordova. Suffering from deep depression, anxiety, and severe paranoia, such suffering led Mr. Domingos to a life of addiction and homelessness. If not for individuals who came and found Mr. Domingos and gave him hope, he may not be here today. Mr. Domingos has lived experience as a homeless person in Rancho Cordova which gives him a unique perspective of what is needed by the homeless in Rancho Cordova.
Also, the inspiration of the Jordan Project is inspired from the passing of Mr. Domingos older brother. Jordan Domingos who was homeless passed away in the city of Bakersfield due to dehydration, Jordan was in search of water on 115-degree day and was unable to find water and passed away.
Such tragedy and experiences are the motivating factor behind The Jordan Project and the (The 200). (The 200's) goal is to ensure that no homeless individual passes away due to a lack of resources but is able to receive basic needs until they are ready to change their lives.
Program:
Per the data from Racho Cordova, currently Rancho Cordova has about 165 homeless individuals within its city limits which has decreased from 300 individuals in 2019. The 200 program would love to serve these individuals. The 200 program will go into the homeless encampments and into the frontline to establish relationships with the homeless in Rancho Cordova and identify their needs. Many homeless programming assists individuals with resources to improve their situation however this is where The 200 program differs. The 200 program will identify homeless individuals needs and serve them regardless of where they are in their process to change. The 200 program will develop a case file for each homeless individual and will learn the services that they are looking for. Once the service is identified, The 200 Program will work on obtaining the resources and bring them into the homeless encampments and to the individual. This is a necessary as this acts of love and service can change hearts increase hope and inspire the homeless individual to improve their situation, as well as give them basic necessities that they are in dire need.
Services:
They types of services that will be provided by The 200 Program to the CEIF APP #121
ITEM 3.1. ATTACHMENT 1
homeless will depend on the client needs however has much experience in the following services: Hot Meal delivery: The 200 program has entered into homeless encampments and has delivered hot meals on site to decrease barriers to access to food. Meaning homeless individuals who are mentally ill or on a binge can still receive a hot meal because it was brought to their locale. Food Drives: The 200 program has purchased food and delivered such food to homeless encampments. Many of the foods are ready to eat and also supply the homeless with the proper utensils to consume the meals in the future. Clothes Drives: Bringing an array of clothes that individuals can choose from to protect them from the harsh climate. Housing Checks: The 200 program ensures and delivers mattresses, blankets, sleeping bags and other items needed to ensure the homeless individuals can be warm and sleep well at night. Lastly; Hygiene:
The 200 program will bring a portable shower to allow homeless individuals an opportunity to bathe and clean themselves. These services will bring hope and will assist the clientele in improving their situation.
Resources:
Lastly, when The 200 program comes across individuals who are at precipice of change and are looking for additional resources, the 200 program will refer such clientele to the appropriate resources that can assist them and improve their situation. Current Partners of The 200 program; Master Care, Victory Outreach (who assists with addiction, mental illness and housing), Rancho Cordova Resource Center will be working alongside all individuals in need of resources to improve their situation and will be referred to other programming if additional help is needed. The 200 program knows that initially that many clients may not seek this resource however with time, delivered resources, and trust built with the client that the number of individuals in search for other resources will increase and will make strides to improving their situation.

Currently, The 200 Program is in its initial stages in serving the homeless population in Rancho Cordova. Currently the 200 program is in search of partners who are willing to donate to the cause of assisting the homeless.
Donations can be physical or monetary. The goal for The 200 program is to increase its donations from partners over this 3-year span in which the donations are able to carry out the services for the homeless population.
The goal is to have local partners continue to assist in the delivery of the services and assist their neighbors and residents in the communities they serve. should any

Unfortunately, (The 200 Program) would not be able to work virtually as it is predicated on delivering face to face, in person services to the most vulnerable population in Rancho Cordova, the homeless. Unfortunately, most of the homeless population do not have access to telephones, computers or any other electronic device that can be used to contact them and deliver services. Due to that hardship are services would be unable to be performed virtually. However, if a public order is passed The 200 program would work and deliver all services following all guidelines given by the public order. The Jordan Project was able to meet safety guidelines during Covid 19 when it was delivering its services to homeless populations.

I have had the privilege of knowing Joshua and Annie Domingos for about five years and have witnessed their unwavering dedication and exceptional contributions to our community.
During our time together, Joshua and Annie have been actively involved in a multitude of community efforts, demonstrating not only their passion for serving others but also their remarkable leadership skills. Whether it was organizing local charity events, leading volunteer groups for community clean-ups, or spearheading fundraising campaigns for important causes, Joshua and Annie has consistently impressed me with their commitment and ability to inspire others.
One notable project that stands out is the Valentines Community Dinner Event took the initiative to spearhead the planning process, meticulously coordinating every aspect of the event.
Joshua was instrumental in engaging the community and encouraging participation. They designed and distributed flyers, utilized social media platforms to spread the word, and personally spoke to residents to invite them to the event. What sets Joshua and Annie Domingos apart is their genuine care for the community and the people within it. They approach every task with enthusiasm, creativity, and a deep sense of responsibility. Their positive attitude is infectious, and they have a remarkable ability to motivate others to get involved and make a difference.

To whom it may concern
My name is Nathan Guerra, and I am the Housing Program Director for Master Care. Master Care is a program that provides services to individuals that are suffering from Chronic health conditions, SMI, SUD’s and facing homelessness. We provide services under the Cal-Aim grant and as of right now, we are in 36 counties across the state and provide services for over 12 mainstream medical Providers. I have been working in the social work field for over 20 years and have helped build outreach teams and strong connections in the community. For the last 10 years I have worked in the county of Sacramento with the Continuum of Care, the veteran’s community, worked for the City of Sacramento and Rancho Cordova.
As a resident of Rancho Cordova that has worked together with City members, directors, multiple agencies, including police forces, I am a firm believer that Josh and Annie Domingo’s and The Jordan Project can play a significant role in helping individuals that are suffering from lack of support of Housing, SMI, and SUD and other barriers. Josh and Annie have the experience, the drive, and the heart to help vulnerable individuals that are suffering due to lack of resources in our community. It is their mission to make a significant impact in the Sacramento region area to help individuals that may be facing barriers and seeking services for success. I passionately believe in and endorse The Jordan Project and will back it up in any way to help build.
The experience I have with the community, resource services and connections will help The Jordan project build a powerful base in our community. I believe that not only does it take individual programs or resources to be successful, but it takes a village. If there is anything that I can do or be the connector to help The Jordan Project make a difference, I am all in for it. I have known Josh and Annie for the last 6 years and have built a relationship with them, and I have seen the heart they have to help others. The Jordan Project can play a vital role in the city of Rancho Cordova and, I believe it would be in our best interest to make sure that we as a community and individuals support in any way to make a difference in individuals one by one.
Thank you for giving me the opportunity to share my heart and support the vision and dreams and mission of someone else so that the legacy of Jordan lives on.
Sincerely,
Nathan Guerra

Project/Program
Description
Sattva California, the home of Positive Mind Performance Training, is a meditation, breathwork and yoga oasis located in Capital Village of Rancho Cordova. Our intention is to offer our Positive Mind Performance Training (PMPT), to schools employees and students within the city, combining mindfulness breathwork and meditation training to reduce stress and anxiety, improve mental health, boost productivity, enhance team communication and cultivate wellness.
We are currently proposing two (2) pilot project sites for school employees and/or students. We have reached out to all of the school districts within Rancho Cordova and they have acknowledged our request and have informed us we are under review for endorsement awaiting results of the grant process.
Training would be in person conducted by certified experts. These classes are practiced sitting on chairs, couches or floor cushions. In addition live streamed classes are provided for days where our trainers are not conducting in-person classes. Once a week In person visits will be coordinated with each pilot site selected. In addition, livestream classes will be provided daily (minimum of five (5) days per week) and available for all participants.
The potential benefit to the community is enormous. All of the major health networks in Sacramento including Dignity, Kaiser, Sutter and UC Davis support and encourage the use of breathwork and meditation to cultivate wellness including mental and physical health. Virtually the entire set of criteria set by the Community Investment and Enhancement Fund is met with this project.
PMPT supports arts, culture and history as a positive outlet for selfdiscovery, inquiry, and enhanced recreational experience. Mindfulness breathwork and guided meditation are key for self-discovery and inquiry.

community in cultivating wellness. We are combining ancient technologies that are time-tested, proven effective and extremely relevant to the needs of school employees and students of Rancho Cordova. We are giving back to the community in these pilot projects with in-kind contributions as part of the funding mechanism.
PMPT supports the growth and development of the mental and physical health of youth in Rancho Cordova.
PMPT founders first moved to Rancho Cordova over 30 years ago and recently established Sattva California, the home of Positive Mind Performance Training in Rancho Cordova to benefit the community and contribute to civic engagement in the most positive manner.
PMPT supports sports teams as a positive outlet for building stronger community connections, promoting physical fitness, and an enhanced participation in sporting activities. Sport centered stress and anxiety among youth is well documented.
PMPT would greatly enhance the quality of educational services to Rancho Cordova school employees, students and residents.
PMPT is committed to Public Safety, having deep roots in the Public Safety Community. One of the founders is Vice-President of the Sacramento Police Foundation and has been involved in youth and school programs on behalf of the Foundation.
PMPT improves Quality of Life as evidenced by all who experience the benefits of mindfulness breathwork and guided meditation. The program provides socio-economic benefit to the City of Rancho Cordova and its employees and residents.
Testimonials from Rancho Cordova residents who have experienced Positive Mind Performance Training include, "This helped save my life" and "thank you for sharing this great experience with others".

Goals
Our goal is to provide two (2) pilot projects for Positive Mind Performance Training to school employees and/or students to reduce stress and anxiety, improve mental health, boost productivity, enhance team communication and cultivate wellness. A positive mind is an intelligent mind conducive to learning, supporting community, promoting healthy physical and emotion well being and increased productivity. A negative mind is a violent mind leading to depression, fear, anger, hatred, and loneliness. We will track participants of the sites chosen for the Positive Mind Performance Training and monitor feedback on the experience of those in the training and the impact on their mental and physical wellness.

Our goal is to provide Positive Mind Performance Training to school employees and students to reduce stress and anxiety, improve mental health, boost productivity, enhance team communication and cultivate wellness.
Sattva California, the home of Positive Mind Performance Training, is a meditation, breathwork and yoga sanctuary located in Capital Village of Rancho Cordova. Our intention is to offer our Positive Mind Performance Training to school employees and students combining mindfulness breathwork and meditation training. Training would be in person at school locations coordinated with each school site at a time appropriate and desired by the school. Livestream classes will be provided daily and available for school employees and students.
The potential benefit to Sacramento City Unified School District is enormous. All of the major health networks in Sacramento including Dignity, Kaiser, Sutter and UC Davis support and encourage the use of breathwork and meditation to cultivate wellness including mental and physical health. A positive mind is an intelligent mind conducive to learning, supporting community and promoting healthy physical and emotional well being. A negative mind leads to depression, fear, anger, hatred, and loneliness.

Instruments
The proposed project would support the purchase of new musical instruments for Cordova High School music program. The Cordova High School music program provides instruments to student musicians when families cannot rent an instrument. Of the 97 Cordova High Band and Orchestra members, only 10 students provide their own instrument currently. Normal wear and tear, even with constant care and repair, shortens an instrument’s lifespan. The current inventory is older, aging and in need of repair and replacement due to being used for many years at marching band practice, football games, parades, traveling, and daily use.
This would fund approximately 24 instruments.

Winter Wind Ensemble
Mandarins proposes a third year of "Immortal Winds”, a winter performing arts ensemble focusing on brass and woodwind instruments, to be based in Rancho Cordova. Winter Winds ensembles present an original show each year, approximately 8–10 minutes in length, and refine it throughout the winter season. Shows are performed and are judged in various musical and visual categories. Immortal Winds will address a current need in the community – high schools in Rancho Cordova only offer winter percussion and winter guard programs and Sacramento State only offers an independent winter percussion group. It will be open to all interested woodwind and brass players between the ages of 16-23 years of age.
Experience camps will be held in November and December during which students will have the opportunity to hone the musical skills and learn basic movements used in the performances. In January, any interested student is eligible to audition for the forty available competitive openings in the ensemble.
