Michael, thank you for meeting with us last month to speak about Operation Splash and being introduced as your new KP grant contact. Congratulations! We are very impressed with the work that your organization provides in the community. You have been invited to submit an application to Kaiser Permanente Southern California Region Community Benefit Charitable Contributions Program in the amount of $60,000 over 2 years (April 2024 to April 2026.)
We look forward to reading your proposal that will include the following:
• Open Swim Passes
Swim Lessons
• Jr. Lifeguard training
Rethink Your Drink
• Senior program (optional)
Please submit your application no later than Wednesday December 13, 2023 using our new application link:
https://mosaic.versaic.com.

If you haven't already, you will need to create a new account to get started (see attachment for step-by-step instructions.) Once you click 'get started', the site will take you to a preliminary routing quiz to begin the application process. When prompted please enter Access code: SCAL_OPSPLASH_2312 As an additional note: I see that we haven't received the final report for the previous Operation Splash grant.

You should have received an email with the link to submit the report. Grant ID# 132215. Kindly submit this report prior to submitting the new proposal, that way we can proceed without delay.

Thank you,
Cecilia Arias, MPH, MCHES
