We are pleased to inform you that a grant of $60,000.00 was approved by Kaiser Permanente to City of San Bernardino for support of Proposal #160838, Operation SPLASH 2024-2026. The initial payment is in process at this time. Subsequent payments are contingent upon timely submission of required reporting and satisfactory progress towards the project goals.
Payment and Reporting Schedule:
Grant Agreement: $60,000.00
Payment 1: $30,000.00 is in process at this time
Payment 2: $30,000.00 scheduled to be disbursed on 3/1/2025, upon review of progress report 2/1/2025, Progress Report submitted within KP's online grants management system. Grantee to report on progress against outlined objectives.
4/1/2026, Final Report submitted within KP's online grants management system. Grantee to report on progress against outlined objectives.
Due to IRS requirements, we will need you to confirm receipt of the payment. After the payment has been issued, you will receive a message with instructions on how to complete this requirement through your submitter portal.
