• The “2006 Plan” was permitted in 2007.
• The permitting culminated with FEMA issuance of a Conditional Letter of Map Revision (CLOMR) on November 30, 2007.
• Project Bid and Construction Contract was awarded on April 14, 2009. • Notice of Completion adopted by City April 12, 2011.
• LOMR requested by the City on December 11, 2011.
• LOMR adopted on June 19, 2012, effective November 5, 2012.

System Ratings
Acceptable System Minimally Acceptable
System
Unacceptable System
All items or components are
rated as Acceptable.

One or more items are rated as
Minimally Acceptable, or one or
more items are rated as
Unacceptable and an
engineering determination
concludes that the Unacceptable
items would not prevent the
system from performing as
intended during the next flood
event.

One of more items are rated as
Unacceptable and would prevent
the system from performing as
intended, or a serious deficiency
noted in past inspections (which
had previously resulted in a
minimally acceptable system
rating) has not been corrected
within the established timeframe,
not to exceed two years.

Recommendations

Page 376 of 382 ends

Floodwall System – Component Item 1
Unwanted Vegetation Growth – Minimally Acceptable
Minimal vegetation growth was observed on the waterside of the floodwall, within fifteen feet of the floodwall.

Recommendation: Remove waterside vegetation within
fifteen feet of the waterside face of the floodwall.

Agenda Item #13.1.

Page 377 of 382 ends

Floodwall System – Component Item 5
Tilting, Sliding or Settlement of Concrete Structures – Minimally Acceptable
The tilting, sliding, and settlement of the “T” wall section at the interface with the “I” wall section just upstream of STA 37+23.50 was observed to be Minimally Acceptable. The tilting, sliding, and settlement of this “T” wall section is evidence that this section of wall has rotated towards Sulphur Creek in relation to the adjacent “I” wall section.

Recommendation: It is recommended that City staff closely monitor this area during and directly after a highwater event and document conditions. It is further recommended that a detailed study of this area be initiated in the nearterm to evaluate the need and extent of bank stabilization required.

Agenda Item #13.1.

Page 378 of 382 ends

Floodwall System – Component Item 7
Monolith Joints – Minimally Acceptable
Over half of the floodwall joints showed a slight widening of the joint and had some type of repair or addition of the sealant at the surface of the joint. Many of these sealant repairs were completed in a way that did not result in the joint sealant being put back to its original condition. In addition, a single location was identified where the top handrail anchors of the baseplate were installed close to a joint in the concrete wall, causing the concrete to pop and spall off the top corner of the joint.

Recommendation: It is recommended that City staff put in place a repair/replacement program for the monolith joints in the floodwall. Further, City staff should regularly inspect the joints for continued deterioration. At the location where the handrail anchors were installed close to a floodwall joint, it is recommended to remove the spalled concrete, coat any exposed reinforcement to prevent reinforcement corrosion, patch the concrete, and resecure the handrail.

Agenda Item #13.1.

Page 379 of 382 ends

Interior Drainage System – Component Item 9
Culverts/Discharge Pipes – Unacceptable
The interior of the drainage culvert that passes through the levee embankment has not been visually inspected or
inspected by video recording equipment within the past five years.

Recommendation: Perform a visual inspection and
provide the report to the inspector for review.

Agenda Item #13.1.

Page 380 of 382 ends

Pump Station System – Component Item 6
Fencing and Gates – Minimally Acceptable
The Concrete Masonry Unit (CMU) perimeter wall (fence) is a Proto II wall system. The CMU Proto II walls on the south side of the pump station were tilting away from the interior of the compound and resulted in a crack at the top of the wall at each of the southeast and southwest corners.

Recommendation: The City staff should monitor the walls on the south side of the pump station on an annual basis for further tilting of the walls towards the inlet. If the cracks at the southeast and southwest corners continue to open further, a more detailed study is recommended.
