As a parent at South School in Hillsborough, I am writing during the 30-day public comment period to formally object to the BHSA Three-Year Integrated Plan as it relates to the proposed funding and development of the Horizon Center facility at 101 North El Camino Real. This email specifically concerns Action Item 1 on the agenda for the upcoming March 4 meeting to "close the comment period" for the BHSA Three-Year Integrated Plan. I formally object to the Behavioral Health Services Act Integrated Plan for Fiscal Years 2026-29 based on the inclusion of the proposed 69-bed facility at 101 N. El Camino Real, referenced on pages 108 and 125 of the plan as the proposed SUD Campus by BHRS contractor Horizon Services. This project represents an unprecedented regional outlier in size, reportedly 300 percent larger than the Bay Area average. It is located within 1,000 feet of schools, daycares, senior living facilities, and residential neighborhoods. To date, there has been no meaningful, site-specific community planning process involving directly impacted community members, stakeholders, or schools. I respectfully request the following: - A 60-day extension of the public comment period, which would require voting against closing the comment period - Establishment of a n Ad Hoc Subcommittee to engage stakeholders
and analyze the site setting - Decoupling of 101 N. El Camino Real from the BHSA Integrated Plan for FY 2026-29 - Identification of an alternative site that is not directly adjacent to schools, childcare centers, senior facilities, or dense residential neighborhoods Pursuant to Section 6.4 of the BHC Bylaws, I formally request that the Behavioral Health Commission establish an Ad Hoc Subcommittee for Horizon Center oversight. The current Three-Year Integrated Plan lists Community Program Planning and Data Review as core pillars. A project of this scale warrants a transparent, data-driven and site- specific review

Dear Members of the Behavioral Health Commission (BHC),

I am a resident of the City of Burlingame and parent of a first grader who attends a school in the City of San Mateo and write to formally object to the proposed Behavioral Health Services Act (BHSA) Three- Year Integrated Plan for Fiscal Years 2026–2029, specifically as it includes the proposed 69-bed Horizon Services facility at 101 N. El Camino Real.

My objection applies directly to the March 4, 2026 “New Business” action items on your agenda SMC BHS Commission Meeting Agenda, including:
 The proposed vote to close the 30-day public comment period on March 5, 2026; and  The proposed vote to submit the BHSA Integrated Plan to the Board of Supervisors.

The draft Plan explicitly references (but only simply) the Horizon project (pp. 108 and 125), describing a campus including sobering beds, ASAM 3.2 detox beds with IMS, and ASAM 3.5 residential treatment beds. This is a comparatively large, high-intensity, 24/7 behavioral health facility proposed in a dense residential corridor within approximately 1,000 feet of schools, daycares, senior living facilities, and established neighborhoods. The following concerns remain unresolved:

1. Insufficient Public Information: To date, there has been minimal disclosure of site designs, operational details, safety protocols, intake standards, transportation management, or neighborhood mitigation measures.

2. Lack of Meaningful Site-Specific Community Engagement: There has been no robust, location- focused community planning process involving directly impacted residents, schools, and businesses prior to inclusion of this project in the Plan.

3. Unprecedented Scale: At 69 beds, this facility is reportedly far larger than comparable Bay Area facilities and represents a regional outlier in size and service concentration.

2 4. Site Sensitivity: The immediate proximity of less than 1,000 feet to an elementary school and that of vulnerable populations such as daycares, senior living facilities, and residential neighborhoods require heightened transparency and analysis.

Closing the public comment period under these conditions would be premature and inconsistent with the spirit of transparent stakeholder engagement required under the Behavioral Health Services Act.

Additionally, moving forward with the as-is proposed BHSA plan for 2026-29 without separating out and excluding just the Horizon 101 N. ECR project puts the entire BHSA plan in collective jeopardy of legal challenge.

Accordingly, I respectfully urge the Commission to consider one or all of the following actions:

ALTERNATIVE ACTION 1: Vote against closing the public comment period and instead vote to extend it for 180 days to allow for meaningful, site-specific review and community engagement before submitting the Plan to the Board of Supervisors. This would also include voting against submitting this plan to the Board of Supervisors.

ALTERNATIVE ACTION 2: Proceed with submission of the current as-is proposed broader BHSA Integrated Plan for Fiscal Years 2026-29 but only if the Commission votes to decouple and exclude the 101 N. El Camino Real Horizon project from the Plan.

In addition to the BHC adopting either Alternative Action 1 or 2, I highly encourage the BHC to establish an Ad Hoc Subcommittee to:
 Assemble and disseminate all project plans and details;  Conduct site-specific review;
 Engage directly impacted stakeholders;
 Evaluate alternative sites;
 Analyze scale, concentration, and compatibility concerns;  Report back with recommendations.

Section 6.4 of the BHC Bylaws empowers the Commission to form such subcommittees where appropriate. A project of this magnitude warrants separate, transparent review — not inclusion within a comprehensive countywide plan without focused scrutiny.

3 The Commission has an opportunity to demonstrate leadership, transparency, and responsiveness.

Excluding or pausing this single project does not undermine behavioral health services countywide — it ensures they are implemented responsibly and with community trust.

Over the past several weeks, the Council has received more than 350 leƩers from residents expressing opposiƟon to this speciﬁc locaƟon. The volume of correspondence reﬂects the seriousness with which the community views this issue.

It appears that the Council in fact received early noƟﬁcaƟon from the County someƟme in 2025 about this proposed project. Despite this early noƟce, no public discussion, community outreach, or noƟﬁcaƟon to residents has taken place.

Why?

Many Councilmembers regularly publish newsleƩers, aƩend neighborhood meeƟngs, and present updates on major developments aﬀecƟng the city. Yet this project - which involves a 69-bed high-acuity detox and residenƟal treatment facility funded almost enƟrely with public funds - was never communicated to the residents of San Mateo unƟl February 2026.

That lack of early transparency by the applicant, the County, and City oﬃcials has contributed signiﬁcantly to the current situaƟon. When a major publicly funded project is advanced without informing the community or engaging local stakeholders, it inevitably erodes public trust. The Council’s inacƟon in this regard represents a serious shortcoming and has contributed to the circumstances we are facing today.

The San Mateo community is compassionate and understands that detox and treatment services are necessary. However, residents will not accept a site selecƟon process that appears to have been driven by secrecy, incomplete informaƟon, no community engageme
nt, and zero visible site analysis that disregards the safety concerns of our children, the elderly, and the general populaƟon.

Given the current situaƟon, I respecƞully ask the Council to take immediate correcƟve steps.

Speciﬁcally, I request that the Council:

1. Agendize a special Council meeƟng
Hold a publicly noƟced special meeƟng dedicated speciﬁcally to discussing the proposed facility at 101 N El Camino Real and act on the proposed items below.

2. Send a formal leƩer to the County and the relevant State grant agency The Council should formally state that no meaningful community engagement occurred prior to mid-February 2026, despite community outreach being a core requirement of the state grant funding that has now been condiƟonally awarded.

Document the sensiƟve uses surrounding the proposed site The leƩer should include a clear inventory of sensiƟve land uses within at least a 1,000-foot radius, including schools, childcare faciliƟes, residenƟal neighborhoods, and senior- serving faciliƟes, so that the applicant, state, and county oﬃcials fully understand the context of the proposed locaƟon.

4. Request to explore an alternaƟve site The Council should request that the County and the State allow the proposed facility to be relocated to a more appropriate seƫng - such as an industrial or medical zone - where safety concerns for vulnerable populaƟons and residents can be minimized while preserving the available state funding for treatment services.

Taking these steps would demonstrate that the City of San Mateo takes seriously both the need for treatment services and the responsibility to ensure that projects of this magnitude are planned transparently and responsibly.

Finally, it is important to recognize that 2026 is an elecƟon year, and Councilmembers will be seeking reelecƟon. The community is paying close aƩenƟon to how the Council responds to this issue, and the posiƟons taken on this maƩer will inevitably inﬂuence how residents evaluate their leadership.

I urge the Council to act promptly and construcƟvely to address the concerns of the community and help guide this project toward a more appropriate and broadly supported outcome.

ALTERNATIVE ACTION 1: Vote against closing the public comment period and instead extend it for 180 days to allow for meaningful, site-speciﬁc review and community engagement before submiƫng the Plan to the Board of Supervisors.

ALTERNATIVE ACTION 2: Proceed with submission of the broader BHSA Integrated Plan for Fiscal Years 2026-2029 only if the Commission votes to decouple and exclude the 101 N. El Camino Real Horizon project from the Plan.

I live two blocks away and this is not the appropriate site for a 69 bed substance abuse and 24/7 sobering facility.

We never received notice of this plan until last week and it seems that this is being ramrodded through to satisfy Gov Newsom’s agenda of adding more beds.

Clearly there must be some better locations in our county for this rather than near several schools and pre schools. Driving south on El Camino past hwy 92, there are several vacant buildings that could serve the need and have less impact on residents that pay a lot of money on property taxes.

I drove around Sunday morning and found a much better property for sale at 1021 S El Camino. It is a larger lot and $1.6M LESS at $4,880,000. Why are taxpayers paying more for a less desirable place??

Please spend our money wisely. This does not align to your fiduciary responsibility for our community.

I am writing as a concerned constituent regarding the proposed Horizon detox and treatment facility at 101 N El Camino Real, which is being advanced by Horizon and San Mateo County.

Given the current situation, I respectfully ask the Council to take immediate corrective steps.

Specifically, I request that the Council:

1. Agendize a special Council meeting
Hold a publicly noticed special meeting dedicated specifically to discussing the proposed facility at 101 N El Camino Real and act on the proposed items below.

Send a formal letter to the County and the relevant State grant agency The Council should formally state that no meaningful community engagement occurred prior to mid- February 2026, despite community outreach being a core requirement of the state grant funding that has now been conditionally awarded.

I am a resident of the City of San Mateo and write to formally object to the proposed Behavioral Health Services Act (BHSA) Three-Year Integrated Plan for Fiscal Years 2026– 2029, speciﬁcally as it includes the
proposed 69-bed Horizon Services facility at 101 N. El Camino Real.

1. InsuƯicient Public Information: To date, there has been minimal disclosure of site designs, operational details, safety protocols, intake standards, transportation management, or neighborhood mitigation measures.

2. Lack of Meaningful Site-Speciﬁc Community Engagement: There has been no robust, location- focused community planning process involving directly impacted residents, schools, and businesses prior to inclusion of this project in the Plan.

3. Unprecedented Scale: At 69 beds, this facility is reportedly far larger than comparable Bay Area facilities and represents a regional outlier in size and service concentration.

Additionally, this location is within easy walking
distance to a full liquor store and the many downtown restaurants and bars that serve alcohol --- all located between 0.2 - 0.6 miles from this proposed location. These folks are already struggling with sobriety and this will be quite a temptation with the in and out privileges this site is going to allow.

Closing the public comment period under these conditions would be premature and inconsistent with the spirit of transparent stakeholder engagement required under the Behavioral Health Services

2 Act. Additionally, moving forward with the as-is proposed BHSA plan for 2026-29 without separating out and excluding just the Horizon 101 N. ECR project puts the entire BHSA plan in collective jeopardy of legal challenge.

Accordingly, I respectfully urge the Commission to consider one or all of the following actions:

ALTERNATIVE ACTION 1: Vote against closing the public comment period and instead vote to extend it for 180 days to allow for meaningful, site-speciﬁc review and community engagement before submitting the Plan to the Board of Supervisors. This would also include voting against submitting this plan to the Board of Supervisors.

In addition to the BHC adopting either Alternative Action 1 or 2, I highly encourage the BHC to establish an Ad Hoc Subcommittee to:
 Assemble and disseminate all project plans and details;  Conduct site-speciﬁc review;
 Engage directly impacted stakeholders;
 Evaluate alternative sites;
 Analyze scale, concentration, and compatibility concerns;  Report back with recommendations.

The Commission has an opportunity to demonstrate leadership, transparency, and responsiveness.

Excluding or pausing this single project does not undermine behavioral health services countywide — it ensures they are implemented responsibly and with community trust.

I am a resident of the City of San Mateo and writing to formally object to the proposed plan to build one of the largest Behavioral Health Facilities in the Bay Area at 101 N El Camino Real in a dense residential corridor without ever getting feedback from nearby residents. It is inconceivable that a project like this could have moved to this point with the majority of the members of the community being totally unaware. Local residents can’t even build a second story on their own property without formal approvals, including notification to or approval from neighbors. Yet a project of this scope is allowed to fly under the radar.

To be clear, I don’t object to the work done by Horizon Health Services. My objection is simply based on the proposed location and the avoidance of public notification to affected residents. I had a younger brother that suffered with mental health and addiction issues. He was treated at one of the Horizon facilities. While he was fine on many days and no one would know anything was wrong with him, there were also volatile time periods where he would be talking to ghosts, ranting about random things from the past that seemed unconnected and scaring many of those around him. There was often no warning when the switch flipped and his demeanor transformed. It is hard to imagine that many of the patients treated at these types of facilities don’t exhibit similar behaviors at times. For people that are unfamiliar with these behaviors, it can be quite scary.

Given the unpredictable nature of these behaviors, it just doesn’t make sense to locate a facility of this type at this location. Children live in this neighborhood. Schools and Daycare Centers are very nearby with even more children. There are also multiple senior living facilities within a few blocks.

It can be a frightening experience for anyone
to encounter one of the patients when they are having a volatile episode, but especially so for children and seniors. These potential problems can be completely avoided by simply building the treatment facility in one of the many suggested more remote locations.

Contemplating a treatment facility of this magnitude in a residential community without full transparency seems to violate the spirit of community engagement. I’d respectfully request that the 101 N El Camino Real location be removed from consideration entirely. Alternatively, notify all residents within a 2-maile radius of the full scope of the proposed plan and only proceed if at least 50% of those residents affirmatively approve the plan.

Follow-up to the SMC Behavioral Health Services Commission Meeting Regarding the Proposed Behavioral Health Facility at 101 N El Camino Real Last night’s meeting of the San Mateo County Behavioral Health Services Commission to ostensibly discuss the proposed plan to build one of the largest Behavioral Health Facilities in the Bay Area at 101 N El Camino Real raised even more concerns/questions than existed before the meeting.
 How could the Board of Supervisors have approved essentially a blank check for $2M without designating a specific site?
 When all the meeting participants were limited to 40 seconds of speaking time (a time far too short by any reasonable standard to make all the salient points), why was the single proponent of the project among the speakers allowed to use her 40 seconds and then move to the table at the end of the meeting and continue making her pitch in an unlimited fashion?
 Amongst the misleading statements made by the proponent was the fact that Horizon Health Services, as an independent business, has the right to purchase and operate in any building they want. While that may be true if they were spending their own money, this is a completely different situation when they are spending taxpayer dollars.
 Why were the concerns raised by around 300 affected residents (dense residential neighborhood, exacerbation of existing traffic congestion, safety of children and elderly, close proximity to establishments that sell alcohol, etc.) almost completely ignored?
 In a community where a resident can’t even build a second story on their own home without formal approval from neighbors, why was a controversial project of this magnitude shrouded in secrecy with a complete lack of transparency?
 What kind of community engagement process do we have around here when County Supervisors can agree to a controversial project like this and City Council Members become aware months ago, but no one bothers to inform the general public and they are left to find out by chance through social media well after the fact? We should be embarrassed as a community that our elected leaders spent more time in the meeting pointing fingers at each other, than actually working to get the answers that people were requesting.  One speaker cited the staggering number of times the police had to be called to a much smaller 17-bed facility. Did anyone even consider that a 69-
bed facility would likely come with an exponential increase in police visits?
 What other locations were considered and why were they rejected? In the meeting, several larger, more cost-effective options were proposed and if the deadline were extended, undoubtedly more suitable locations could have been identified.
 Why was there no opportunity for residents to ask questions in the meeting when most just found out about the project a few days before?
Given the unpredictable nature of the behaviors of the patients, it just doesn’t make sense to locate a facility of this type at this location near families, schools, day care centers and senior living facilities when there are many alternate

2 locations that are more cost effective and safer for all involved. The potential unintended consequences are too high. Considering there is so much valid local opposition to this location, please reconsider the decision to move forward. Thank You.

Unfortunately, your response confirms what many residents already fear: the County is refusing to allow a simple public Q&A where the community can hear questions and answers openly regarding the proposed facility at 101 N. El Camino Real.

Instead, the County has chosen a so called “open house” that is really a mask for a fragmented “divide and diffuse” comment-gathering format where residents are expected to move from table to table speaking privately with staff and project representatives.

That format may be a convenient defensive measure for the County, but it avoids the transparency the community has repeatedly asked for.

When answers are given privately at different tables:

The community cannot hear the same answers together
There is no shared understanding of what is being said
There is no public accountability for the information being provided

This appears purposefully divisive and dismissive and that is cause for grave concern. For a $27 million publicly funded project proposing a 69-bed detox and residential treatment facility in the middle of an established neighborhood, requiring, and expecting a 45-minute public Q&A is not only reasonable – it is warranted - required
. It is the bare minimum level of transparency residents deserve.

It is especially troubling given that the County’s $2 million funding commitment for this project was originally approved quietly under a Consent Agenda item with no discussion and virtually no public awareness.

Given that history, residents are simply asking for something very basic now: open questions and open answers in front of the community.

Instead, the County has chosen a format that fragments the discussion and prevents the public from hearing the full conversation.

This issue is not about opposing treatment services. Many residents support expanding treatment resources in our county.

The issue is transparency, responsible site selection, and a public process that respects the community affected by the decision.

Residents will attend the March 24 meeting, and we will continue to ask the same simple questions:

Why is the County unwilling to allow the community to hear questions openly and hear answers together in a transparent public forum?
Why is the County unwilling to move this planned large drug detox facility with up to 17,520 drop-offs per year to a location more suitable away from an elementary school less than 1,00 ft. away?
Why is the County unwilling to publicly disclose relevant details in an open and transparent manner and to provide meaningful information for public consumption?

The community deserves better.

Dear Members of the Behavioral Health Commission (BHC),

I am a resident of the City of San Mateo and write to formally object to the proposed Behavioral Health Services Act (BHSA) Three-Year Integrated Plan for Fiscal Years 2026-2029, specifically as it includes the proposed 69-bed Horizon Services facility at 101 N. El Camino Real.

My objection applies directly to the March 4, 2026 "New Business" action items on the San Mateo County Behavioral Health Commission Meeting Agenda, including:

- The proposed vote to close the 30-day public comment period on March 5, 2026; and - The proposed vote to submit the BHSA Integrated Plan to the Board of Supervisors.

schools, daycares, senior living facilities, and established neighborhoods.

The following concerns remain unresolved:

Unprecedented Scale: At 69 beds, this facility is reportedly far larger than comparable Bay Area facilities and represents a regional outlier in size and service concentration.

Closing the public comment period under these conditions would be premature and inconsistent with the spirit of transparent stakeholder engagement required under the Behavioral Health Services Act. Additionally, moving forward with the proposed BHSA Plan for 2026-2029 without separating and excluding the Horizon project at 101 N. El Camino Real places the broader Plan at potential risk of legal challenge.

Accordingly, I respectfully urge the Commission to consider one or both of the following actions:

ALTERNATIVE ACTION 1: Vote against closing the public comment period and instead extend it for 180 days to allow for meaningful, site-specific review and community engagement before submitting the Plan to the Board of Supervisors.

ALTERNATIVE ACTION 2: Proceed with submission of the broader BHSA Integrated Plan for Fiscal Years 2026- 2029 only if
the Commission votes to decouple and exclude the 101 N. El Camino Real Horizon project from the Plan.

In addition, I strongly encourage the Commission to establish an Ad Hoc Subcommittee to assemble and disseminate all project plans and details, conduct a site-specific review, engage directly impacted stakeholders, evaluate alternative sites, analyze scale, concentration, and compatibility concerns, and report back with recommendations.

Section 6.4 of the BHC Bylaws empowers the Commission to form such subcommittees where appropriate. A project of this magnitude warrants separate, transparent review.

I respectfully request that this email be entered into the public record for the BHC Meeting on March 4, 2026.
